Case Manager Case Management Full Time 81657BR Job Summary The Case Manager provides services to the Emergency Department. The Case Manager promotes the achievement of optimal clinical and resource outcomes and is responsible for facilitating appropriate lengths of stay for all hospital admissions in accordance with its goals and objectives. The case manager is responsible for a designated patient caseload that is considered complex and resource intensive, and oversees coordination of complex discharge planning for assigned case load. The Case Manager is an active member of the interdisciplinary team contributing to team goal setting. The Case Manager acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $73.82 - $100.64 (Hourly Rate). Master's degree preferred. Preferred Qualifications * Recent experience in case management, utilization review or discharge planning preferred.
