Overview

Now hiring an Oncology Navigator Coordinator!

Performs visit closure activities to coordinate patient care while providing an exceptional patient experience for successful completion of next steps ordered by the clinician
Advanced scheduling for specialty or radiologic procedures
Recognizing payor requirements and obtaining prior authorization/certification/referrals for visits/procedures
Processes daily referral workqueues for authorization, deferring only those that require payor consideration
Primary Care and most Specialty Care offices must process STAT and ASAP referral workqueues for results received
Return scheduling knowledge to include the ability to scrub schedules for accuracy and efficiency in home department
Monitor in-baskets and reports for overdue results, waitlisted appointments, recalls, etc

Qualifications

  • 2 + year of Healthcare experience
  • BLS
  • Will require a Lay Navigator certificate within 6 months of hire.
  • Demonstrate an ability to communicate effectively in person and via telephone with members, employer groups, physicians and physician offices using precise language and being customer focused.
  • Strong written communications.

Education Essential: High School Diploma or GED

BLS Certification required at time of hire.



Responsibilities

  • This role is both clinical and clerical, requiring interactions with patients and other health care providers.
  • Prepare and gather medical records (Pathology & Imaging) requested for new patient on-boarding and education. They will then scan to the appropriate Navigator.
  • Participant of the Interdisciplinary and multidisciplinary care plan team, to ensure appropriate interventions, cost effective delivery of quality care and services across the continuum.
  • Assist Nurse Navigation team with assessment of oncology patient needs and coordination of plan of care regularly to identify gaps in care, trends to improve health and quality of life outcomes.
  • Assist nurse navigation team in and Implementation of risk assessments, healthcare and quality of life pathways for cancer patients, based upon the clinical staging of the patient. Evaluates progress of patients along the continuum and is responsible for professional and personal modifications along with healthcare team as appropriate.
  • Assist Navigator with the initiation of education prep documents and schedules patient with Navigator.
    *Responsible for data collection of: clinical path variance data that indicates potential areas for improvement of case and services provided; works with members and the interdisciplinary and multidisciplinary care plan team to adjust plan of care, when necessary and with tracking of metrics and quality indicators for diagnosing and treating cancers promptly and effectively.
  • Provides lay navigation to oncology patients with stable and chronic condition and with those whose care requires coordination for some period of time.
  • Responsible for authorization of services, and serves as point of contact to ensure services are rendered appropriately.
    *Under the supervision of nurse navigation team will conducts health risk assessment and/or comprehensive needs assessment which includes, but is not limited to psycho-social, physical, medical, behavioral, environmental, and financial parameters with appropriate referrals to the different disciplines.
  • Identifies clinical patient care needs and directs them to the Navigator.
  • Provides access to community resources and links patients to support services such as: social work, nutrition, psychologist, pastoral care, physical therapy and rehabilitation, financial counseling, integrative medicine, pharmacy, legal services, palliative care, hospice, etc.
  • Supports nurse navigation with implementation and coordination of plan of care for patients and families to improve health and quality of life outcomes.
  • Acts as an advocate for patients and families by identifying and addressing gaps in care.
  • Develops relationships with cancer physicians and APCs, including but not limited to: Radiologist, General Surgeon, Surgical Oncologist, Medical Oncologist, Radiation Oncologist, Pathologist, Plastic Surgeon, Primary Care Physician, Palliative Care, Hospice and serves as liaison between patient and physicians.
  • Develops relationships with supportive care team including but not limited to Research , dietician, social workers, financial advocate, triage nurses, infusion nurses, and clerical support team
  • Assist nurse navigator and care team with preparation of and coordination of the weekly cancer multidisciplinary conference.
  • Assist the Navigator with initiation, prep and scheduling of treatment summaries/survivorship care plans.
  • Responsible for multidisciplinary conference: e-mail, communication, setting up and take down of preparing conference rooms.
  • Will assist the nurse navigation team with collaboration with the research team regarding opportunities for referral to clinical trials.
