If you are a Registered Nurse looking for a refreshing change of pace where you can step away from hospital chaos, build lasting relationships, and make a profound difference in people's daily lives, Mosaic welcomes you!

We are seeking a dedicated, compassionate Registered Nurse (RN) to oversee the health and well-being of individuals with intellectual and developmental disabilities across our community residential homes in Pontiac and Dwight. Here, you aren't just managing tasks on a floor-you are empowering individuals to live meaningful, healthy lives in warm, home-like environments.

Why You'll Love Nursing with Mosaic:

  • True Autonomy & Variety: No two days are the same! Manage your schedule while enjoying independent clinical decision-making without feeling micromanaged.

  • Manageable Pacing: Experience a far less overwhelming atmosphere compared to acute hospital or long-term care facilities.

  • Deep, Meaningful Relationships: Get to know the people you support on a personal level. Building trust and watching individuals thrive under your care is the most rewarding part of the job.

  • You're Never on an Island: Enjoy direct support from a collaborative local team (Area Director, Health Services Assistants) and connect with a strong regional and national network of peer nurses.

What Your Day Looks Like:

As our RN, you will serve as a clinical partner, health advocate, and educator. Your primary focus is person-centered health oversight:

  • Health Assessments & Oversight: Perform monthly health assessments, monitor changes in condition, review health records, and follow up on physician orders.

  • Collaborative Care: Partner with doctors, specialists, and StationMD virtual care to advocate for optimal health outcomes for each person served.

  • Education & Empowerment: Train and empower direct support staff on medication administration, health protocols, and specialized clinical care (such as G-tube management, insulin, and GLP-1 injections).

  • Person-Centered Advocacy: Accompany individuals to key medical appointments as needed, ensuring their voices and choices are heard.

  • Documentation: Maintain accurate, timely medical notes within the Therap electronic health records (EHR) system.

Work Environment & Schedule:

  • Primary Office: Pontiac, IL.

  • Community Settings: Supporting small, comfortable residential homes and duplexes in Pontiac and Dwight.

  • Hours: Standard daytime schedule (typically 8:00 AM - 4:30 PM, M-F) offering great work-life balance.

  • Shared On-Call: 1 week ON / 1 week OFF shared with a peer nurse to field after-hours assessment calls or triage changes in condition. (On-call time is compensated at your active hourly rate when responding).
