JOB SUMMARY Manage the restorative program in Panorama's Convalescent and Rehabilitation Center. Ensure quality care that promotes the highest attainable physical, mental, and psychological well-being of each resident as determined by resident assessments and individual plans of care. This position requires collaboration and thorough communication with Director of Nursing. Manage the facility Wound Rounding Program with United Wound healing, communicating new facility need and carrying out recommendations from UWH provider. DUTIES AND RESPONSIBILITIES: Restorative Program Management Design, implement, and monitor the restorative program for all residents. Develop individualized care plans from therapy recommendations and functional assessments, including carryover when residents discharge from therapy. Manage program rosters for ambulation, ROM, ADLs, splinting, contracture care, feeding, and bladder retraining. Oversee documentation systems, flow sheets, and monthly progress evaluations. Coordinate procurement of adaptive equipment and assistive devices. Staff Leadership and Collaboration Supervise, train, and provide direct oversight of Restorative CNA staff and floor CNA staff when needed. Collaborate with Charge Nurses, Therapy, Nursing, and Dietary so that restorative plans are followed, and residents maintain their highest level of function. Direct Restorative Care Observe and assist residents with eating, swallowing, feeding techniques, and use of adaptive equipment. Apply and monitor splints, braces, and positioning devices as ordered by physicians or therapists. Oversee restorative dressing and grooming to promote resident independence. Perform restorative ambulation, transfers, and active/passive range of motion exercises. Wound Care Coordination Serve as facility Wound Care Nurse and as liaison between the facility and United Wound Healing (UWH) to coordinate wound treatment plans, provider visits, and follow-up care. Round with the UWH provider weekly and as needed, and address concerns as they arise. Communicate new admissions and facility needs to the appropriate UWH representative. Ensure UWH recommendations are accurately transcribed into physician orders. Compliance Conduct special compliance audits for the facility as assigned. EDUCATION/EXPERIENCE: Must possess an unencumbered Washington Registered Nurse license. Experience in Long Term Care required. Experience with restorative nursing preferred. Experience supervising others required. Preferred wound care certification or experience managing wounds.
