JOB SUMMARY
This position works with the Director of Nursing, Nurse Manager, Clinical Supervisors, clinical team and office team to support workflow functions that impact chart review. Review all OASIS assessment documents for accuracy, consistency and supporting documentation. Review clinical charting for conformance/compliance to Medicare and Agency standards. Discuss any noted documentation discrepancies with Supervisors and clinical staff as appropriate. Participate in quality assurance and improvement activities for the agency.

  (RN) REGISTERED NURSE by Board of Nurse Examiners for the State of Texas Upon Hire or
  (NLC) NURSING LICENSURE COMPACT-BNE - (BNE Compact State) Upon Hire

JOB DUTIES

• Reviews OASIS assessment documents for accuracy, consistency and supporting documentation.
• Ensures that the clients plan of care is executed as written.
• Audits daily notes for proper documentation and ensures chart reviews are timely, accurate, and meet all data requirements under CMS guideline.
• Monitors compliance with both internal standards and regulatory requirements. Develops and implements appropriate plans of action to correct identified deficiencies as needed.
• Discusses any noted documentation discrepancies with Supervisors and clinical staff as appropriate.
• Regularly participates in case conference activities and assures through attendance and record reviews that conferences are conducted and documented in accordance with Agency policy.
• Reviews all reports of complaints, errors, incidents, and grievances to ensure that they are fully documented and properly handled in the timeframe and manner specified by the Agency's policy.
• Performs internally defined audits of operational activities in accordance with appropriate accrediting standards and prescribed departmental policies.
• Participates in development and implementation of a staff educational program for documentation and Quality Improvement (QAPI).
• Performs other duties as assigned.
