Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to join its HouseCalls program, delivering annual in-home health assessments to Medicare Advantage and other health plan members. This field-based, non-prescribing role focuses on preventive care, identifying health risks, closing gaps in care, addressing social determinants of health, and collaborating with primary care providers to improve member outcomes in a home-based setting. Position Highlights
  • $40,000 Student Loan Repayment OR $20,000 Sign-On Bonus
  • Flexible Scheduling - No call, weekends or holidays (flexible provider schedules available between 20-40 hours per week)
  • Conduct comprehensive in-home health assessments for Medicare Advantage and health plan members.
  • Perform physical exams, medication reconciliation, screenings, and preventive evaluations.
  • Identify and document diagnoses that support care management and treatment planning.
  • Collaborate with primary care providers to address care gaps and improve outcomes.
  • Educate members on health conditions, medications, and available community resources.
Required Qualifications
  • Active ANCC/AANP certification for Nurse Practitioners or NCCPA certification for Physician Assistants.
  • Active, unrestricted state licensure (or ability to obtain prior to start date).
  • Active, unrestricted driver's license and reliable transportation for home visits.
  • Ability to work in varied home environments and perform physical assessment activities as needed.

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
