Purpose:


This position is responsible for the daily management of departmental operations for the Hospital Care Management Department. The Manager is accountable for the implementation of strategies and initiatives developed by the Director of Care Coordination. The Manager provides staff support and oversight of activities related to Case Management, Social Worker, CDI and UR functions. This position is also accountable for the hiring, developing and coaching of departmental staff. The Manager may also be called upon to participate in committees, develop or provide input into process improvement, and monitor staff performance with the goal of achieving performance goals and metrics.



Nature and Scope:

This position is responsible for the operation, i.e., direction and daily management of Hospital Care Management clinical and non-clinical staff and has the authority to direct, counsel, evaluate, discipline, schedule and participate in the hiring process.The Manager facilitates the following Case Management objectives: (1) Coordination of ancillary and specialty service referrals 2) Collaboration with clinicians and other stakeholders to ensure integrated departmental operations 3) facilitation of seamless patient transitions of care across the system (4) ensuring compliance with state and governmental regulations and (5) knowledge of utilization review processes and payer contractual requirements related to Utilization Review. This position does not provide patient care.

The Manager makes no clinical adverse determinations. Incumbent is responsible for assisting in the design and continuous improvement of the care management processes under the direction of Hospital Care Management Leadership. This position directs the daily operation and function of case management, which includes:

* Oversight and development of data collection, including tracking and trending outcomes.

* Interdisciplinary collaboration with Renown Regional Medical Center and Renown South Meadows departments and management.

* This position is responsible for the operation, i.e., direction and daily management and has the authority to direct, counsel, evaluate, discipline, schedule and participate in the hiring process.

* This position acts as a resource for appropriate disposition of clinical services, utilizing the Director of Hospital Care Management for assistance and direction with difficult or problematic cases.

* This position collaborates with Physicians, Nursing, Ancillary Staff, Patients, Families, Post -Acute Providers and Revenue Cycle to ensure staff are performing their duties and optimal level.

KNOWLEDGE, SKILLS & ABILITIES

1.) Knowledge of case management practices and ability to incorporate successful current trends.

2.) Leadership skills, which model problem resolution, sound decision-making, consensus building, and effective resource utilization

3.) Ability to plan, organize, adjust and maintain priorities and schedules to set and meet deadlines.

4.) Ability to develop, understand, and monitors budgets.

5.) Computer familiarity and literacy. Experience in operating database and/or spreadsheet programs.

6.) Knowledge and understanding of governmental and JCAHO regulations.

7.) Strong verbal and written communication skills. This position does not provide patient care




Minimum Qualifications: Requirements - Required and/or Preferred






Name Description
Education: Ability to read, write, speak, and understand English sufficiently to perform job duties safely and effectively. Bachelor of Science in Nursing preferred.
Experience: Two years' experience as a Clinical Leader in home health, long-term care, acute care, rehabilitation, or utilization review required. Demonstrated experience in a managerial, supervisory or leadership role preferred. Preference will be given to candidates with case management experience in two or more service areas, and candidates with clinical experience.
License(s): Ability to obtain and maintain a State of Nevada Registered Nurse license or a Registered Nurse License in the state at which they reside is required.
Certification(s): National Certification preferred (i. e. , Case Management (CCM))
Computer / Typing: Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel, Teams, and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
