At RWJBarnabas Health, our Home Infusion Registered Nurses bring specialized nursing care directly to patients, helping individuals and families safely manage complex therapies in the comfort of home. As an Infusion RN, you'll work independently while remaining closely connected to physicians, pharmacists, caregivers, and other members of the healthcare team. From administering infusion therapies and managing vascular access to providing hands-on education and clinical monitoring, you'll help adult and pediatric patients confidently navigate their treatment outside of the traditional hospital setting. This role offers an opportunity to combine advanced infusion skills, autonomy, patient education, and relationship-centered nursing to make a meaningful difference throughout each patient's treatment journey.

As a Home Infusion Registered Nurse (RN), a typical day may include:

  • Traveling to patients' homes to provide skilled infusion nursing services while independently managing an assigned patient schedule
  • Performing comprehensive assessments before, during, and after infusion therapy and communicating changes in condition or adverse findings to physicians and pharmacy team members
  • Administering IV infusions, injections, chemotherapy, biologics, parenteral therapies, and first doses of antibiotics according to physician orders and established protocols
  • Establishing and maintaining vascular access, including peripheral IVs, PICC lines, ports, and central lines, while identifying and managing access-related concerns
  • Educating adult and pediatric patients and caregivers on infusion administration, pump operation and troubleshooting, medications, potential side effects, and signs and symptoms of complications
  • Assessing each patient and caregiver's ability and willingness to safely participate in home infusion therapy and adapting education to their individual needs
  • Initiating continuous home infusion therapies in physician offices, outpatient clinics, hospital settings, and patient homes while obtaining required consents and beginning patient education
  • Monitoring patients for adverse reactions and responding appropriately to clinical concerns and emergencies
  • Performing skilled nursing visits based on individual treatment plans, including blood collection and coordination of laboratory specimen delivery when clinically indicated
  • Maintaining infection prevention, sterile technique, medication safety, proper sharps and hazardous waste handling, and established infusion best practices
  • Accurately documenting assessments, treatments, education, interventions, and patient responses within the electronic medical record
  • Collaborating with physicians, pharmacists, patients, caregivers, and other healthcare professionals to promote safe, coordinated, and patient-centered home infusion care

This role might be for you if:

  • You are confident working independently and making sound clinical decisions outside of a traditional hospital environment
  • You have strong IV therapy and venous access skills and enjoy using your clinical expertise to manage complex infusion treatments
  • You enjoy teaching patients and caregivers and can translate detailed clinical instructions into clear, understandable guidance
  • You are comfortable providing age-appropriate care and education to both adult and pediatric patients
  • Strong assessment and critical-thinking skills allow you to recognize changes in patient condition and respond appropriately
  • You are highly organized and comfortable independently managing patient visits, scheduling, documentation, equipment, and changing priorities throughout the day
  • You value building trusting relationships with patients and families while helping them become more confident managing therapy at home
  • You communicate effectively with physicians, pharmacists, caregivers, and other members of the healthcare team
  • You bring attention to detail and a strong commitment to infection prevention, medication safety, regulatory compliance, and quality improvement
  • You enjoy the autonomy of home-based nursing while remaining connected to a collaborative clinical team

To be considered for this Home Infusion Registered Nurse opportunity, you must be a graduate of an accredited nursing program with an Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN), hold an active New Jersey Registered Nurse (RN) license, and maintain current BLS and IV certifications. Candidates should possess a minimum of two to three years of RN experience, preferably within infusion therapy, medical-surgical oncology, ambulatory care, or critical care, along with demonstrated experience in IV therapy and venous access. Previous home care experience is preferred. Candidates must have a valid driver's license, current automobile insurance, and reliable transportation to complete patient home visits. The ideal candidate demonstrates strong independent clinical judgment, assessment, communication, patient and caregiver education, vascular access, organization, and critical-thinking skills. Candidates must be comfortable providing infusion care to both adult and pediatric patients and fulfilling applicable on-call and holiday coverage requirements. This is a salaried day shift opportunity supporting Home Infusion services through LSC-Montrose.
