Your Role on the Team

Position Summary:

The Bridges Coordinator is responsible for the comprehensive coordination and oversight of newborn family screening and risk assessment within the assigned facility, utilizing the Bridges Maternal Child Health Screening Tool to identify families who may benefit from additional support services. The incumbent maintains accountability for the completion of prescreening, eligibility determination, and timely referral to enhanced services, including home visitation programs and CalWORKs Home Visiting Program, ensuring all documentation and data entry meets program timelines and data integrity standards. This role provides non-clinical bedside education and support to postpartum families, addressing available community resources for breastfeeding, immunizations, newborn care and postpartum wellbeing, while identifying and facilitating referrals for families with social, developmental, behavioral health, or psychosocial risk factors. The Coordinator manages inventory and distribution of educational materials and Kits for New Parents, ensures compliance with data privacy, HIPAA, consent, and information-sharing requirements, and serves as the hospital's liaison with First 5 Orange County / Bridges program leadership, participating in meetings, audits, training, and program improvement initiatives. The incumbent collaborates closely with nursing, case management, social work, physicians, lactation consultants, and community partners to support seamless care transitions, educates hospital coworkers on program workflows and resources, and monitors program performance metrics to ensure contract compliance and successful achievement of outcomes. This position exercises independent judgment in integrating screening and referral processes into routine, non-clinical maternal-child operations and supports regulatory compliance activities as needed.

Required Qualifications:
  • Strong interpersonal communication skills, with the ability to engage patients respectfully and in culturally responsive ways.
  • Skill to communicate and interact effectively and pleasantly with colleagues, faculty, students, and community leaders, from multiple cultures.
  • Possible weekend and evening hours
  • Nonjudgmental, energetic, positive, harm-reduction approach to assisting patients with SUD with respect for patient confidentiality and privacy.
  • Greater than 2 years of data science research and/or work experience.
  • Flexibility, adaptability, and resilience in a clinical setting with shifting priorities.
  • Experience with electronic documentation, reporting systems, and data tracking
  • Demonstrated skill in data management and analysis
  • Broad knowledge of patient experience concepts and industry trends
  • Ability to work in a dynamic environment of rapidly changing priorities and frequent interruptions.
  • Ability to use electronic health records and standard office software (e.g., Microsoft Office, Outlook); willingness to learn internal data and documentation systems.
  • Ability to successfully represent UC Irvine Health to the external community
  • Ability to analyze and take action on complex matters, documents, and tasks. Ability to take the initiative and independently respond to requests in writing, in person, or over telephone.

  • Preferred Qualifications:
  • Strong documentation skills, including data entry, file organization, and written communication.
  • Strong communication, teaching, interdisciplinary collaboration, and relationship-building skills
  • Knowledge of community resource navigation, perinatal mental health, and family support services
  • Experience in community collaboratives and marketing in the health care arena
  • Ability to communicate in languages spoken in the local community is a plus.
