What You Will Do: This is a dynamic role for an expert Case Manager to manage an assignment of complex cases on adult floors of The Johns Hopkins Hospital. You will work closely with Social Worker, Physician advisor, and Care Coordination leadership to ensure quality and efficient coordination of care.In collaboration with patients/families, Care Coordination leadership, physicians, interdisciplinary team, the Assistant Nurse Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. Manages clinical resources and oversight of operations related to transition planning for patients and actively working to ensure we are identifying/ eliminating barriers to the delivery of clinical services with the patient, family, and care team. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
