Location: Mostly Remote (Includes Local Home Visits) Schedule: Monday - Friday, 8:30 AM - 4:30 PM (plus rotating on-call schedule) Are you passionate about making a real difference in your community? What You'll Do * Coordinate & Support: Receive new referrals, manage intakes, and create personalized service plans for home and community-based services. * Connect in the Field: Conduct regular home visits to ensure direct care quality and support standards are met. * Build Relationships: Partner closely with clients, case managers, direct care workers, and community partners. * Maintain Operations: Oversee scheduling adjustments, handle incident investigations, and provide rotating after-hours on-call support.
