Position Summary

The Program Manager plans, organizes, directs and administers the residential care and support services of the Alegria RCFCI transitional housing program, to include staffing, nursing coordination, clinical support, clerical support, compliance and contract administration.

Essential Function



  • Plans, coordinates and directs the work of all direct care staff and contractors, including certified nursing assistants, the registered nurse, therapist, and other support staff allocated to the RCFCI.
  • Maintains and ensures top-tier care of all residential families per Alegria.
  • Establishes and maintains appropriate policies, procedures, rules, regulations, and statutes as required by accrediting, contracting and auditing agencies.
  • Coordinate's care services and participates in all individualized service planning meetings and case conferences
  • Provide the follow up documentation of communication with clients who have existed the RCFCI within the last 18 months.
  • Obtain a working knowledge of HMIS / E2LAand manage HMIS/E2LA database as it pertains to client services and reporting standards.
  • Facilitate individual service plan and case conference meeting with the Program manager, social worker/ therapist and RN/case manager for clients in the RCFI who are in the discharge phase.
  • Works in conjunction with the direct care staff for admission, discharge, eviction, and treatment planning.
  • Participates in initial and on-going assessments of client's wellbeing and eligibility for program
  • Acts as liaison between state licensing/contracting agencies and Alegria.
  • Ensures compliance of all personnel management - including scheduling, record keeping, time-activity reports and performance evaluations of all direct care staff and contractors.
  • Conducts appropriate and comprehensive follow-up to all incidents and issues relative to resident health, safety and welfare.
  • Serves as part of the Leadership and Quality Management Team; working in cooperation with other members to coordinate the effective development of facility resources and implement resident care services designed to meet the program goals.
  • Maintains community outreach in designated community districts in order to enhance services and maintain program utilization.
  • Oversees program evaluation and quality improvement activities in coordination with the Leadership Team.
  • Assists in the development of departmental budget and monitors expenditures.
  • Ensures appropriate and regular utilization of required databases, such as HMIS and Case Watch
  • Maintains on-call status on a rotating basis.
  • Utilizes assessment tools that support data collection and reporting.
  • Regularly audits program files to ensure compliance.
  • Documents all contacts with resident and team members regarding resident's case in the resident's file.
  • Provides or organizes in-service education to staff regarding the care and supervision of people with HIV/AIDS and on universal precautions.
  • Assists in coordinating HIPAA compliance activities.
  • Performs other duties as assigned.


Minimum Qualifications



  • Bachelor of Social Work or equivalent at minimum 2 years' experience working/volunteering in social service setting.
  • Master's degree in social work strongly preferred.
  • Experience working with mentally ill, HIV/AIDS and homeless population preferred.
  • Must pass criminal background check
  • Bilingual (English/Spanish) is preferred.


Skills, Knowledge & Abilities



  • Must be culturally sensitive and non-judgmental
  • Knowledge and sensitivity regarding issues of homelessness and/or HIV/AIDS
  • Drive test and clean MVR check
  • Ability to work with diverse populations in a caring, sensitive, and non-judgmental manner
  • Acquired case management skills that include the ability to conduct an assessment and provide appropriate referrals
  • Knowledge of social services practices, contract outcome, and community care licensing regulations,
  • CPR/First Aid Certification

Qualifications
Experience
Bilingual (English/Spanish) is preferred. (preferred)
