Northeast Medical Group (NEMG), a member of Yale New Haven Health (YNHH), is seeking a Per Diem Palliative Care Physician to provide inpatient consultative services at Greenwich Hospital.

This position focuses on delivering high-quality, patient-centered palliative care to hospitalized patients with serious and complex illnesses. The physician will work collaboratively with an interdisciplinary care team to manage symptoms, support goals-of-care discussions, and improve quality of life for patients and their families.

Key Responsibilities

  • Provide inpatient palliative care consultations and follow-up care for hospitalized patients
  • Perform comprehensive clinical assessments, including symptom burden, prognosis, and care needs
  • Manage complex symptoms including pain, dyspnea, nausea, and psychosocial distress
  • Facilitate goals-of-care discussions, advance care planning, and end-of-life decision-making with patients and families
  • Collaborate with interdisciplinary teams (nursing, social work, chaplaincy, case management, and primary teams) to develop and implement individualized care plans
  • Participate in family meetings to provide education, support, and alignment around care goals
  • Coordinate care across the hospital setting to ensure continuity and appropriate transitions of care
  • Document clinical encounters, treatment plans, and patient progress in the electronic medical record
  • Provide education and guidance to hospital staff on palliative care principles, when appropriate

Qualifications

  • Qualifications

    • MD or DO from an accredited medical school
    • Board Certified/Board Eligible in Hospice and Palliative Medicine (or eligible pathway)
    • Board certification in Internal Medicine, Family Medicine, or related primary specialty
    • Active (or eligible) Connecticut medical license
    • Demonstrated expertise in symptom management and goals-of-care communication
    • Strong interpersonal and collaboration skills with the ability to work effectively in a team-based environment

    Schedule

    • Per diem, inpatient coverage - Monday - Friday, Days
    • Flexible scheduling based on program needs
