Description

This is a member of a multidisciplinary team responsible for the evaluations and treatment of SIU Cleft and Cranial patients. Primary responsibility of the position is to ensure that safe, quality, efficient health care delivery is provided to patients by serving as a patient service coordinator within the Departments and providing direct nursing services and support for surgeons to which he/she is assigned.

Examples of Duties

Hospital Duties 50%
A. Demonstrates, by actions, commitment to the mission and the behavioral standards of SIU School of Medicine. Provides excellent service to both internal and external customers through collaboration and partnership; compassion and respect; integrity and accountability; diversity and inclusion; as well as continuous learning and improvement.
B. Makes rounds on patients at both hospitals. This includes:
1. Speaking with patients/families on current and future care
2. Reviewing patient's records for lab, x-ray, physician and nurses' notes
3. Writing factual progress notes in the patient's chart using identifying signature
4. Recording physician's orders in the medical record, as given by the supervising surgeon
5. Interviews patients and their families to determine patient/family dynamics, needs, and identification of any contributing factors to illness; guides other staff members in patient teaching; and assists patient's family to participate in patient's care plan.
6. Coordinates follow-up of outpatient services among multiple specialties for each individual patient. Facilitates scheduling and performance of tests and procedures across multiple SIU departments and affiliated institutions. Provides follow-up care in conjunction with attending physician/nurse practitioner/physician assistant furnishing information and teaching.
7. Check in on post-operative patients, inspecting wounds and/or changing dressings

Nursing Clinic Duties 30%
A. Acts as a liaison between healthcare facilities and other specialty agencies.
B. Provides clinic nursing care and delegation of clinical duties to LPN's and/or MA's while in clinic. This care may include but not be limited to obtaining patient histories, assisting with portions of physical examinations, administering medications, providing wound care, performing other nursing procedures and documenting nursing functions appropriately.
C. Assists with procedures. Sets up and/or completes procedures as necessary under the direction of the MD, APN, or PA
D. Develops and implements patient plan of care with the MD, APN, and/or PA
E. Supports the clinical efforts of the SIU Cleft Clinic as needed through patient/family teaching, clinical management, discharge planning, etc.
F. Schedule follow-up patient activities including radiology studies, surgical procedures and other testing as ordered by the MD, APN or PA
G. Prepares for upcoming clinics by obtaining history from referring physician, pertinent radiologic studies, laboratory results, and operative reports when appropriate; prenatal and birth history
H. Communicates with patients, families and other healthcare professionals
I. Uses critical thinking skills to triage patient care priorities for patient phone calls and needs
J. Ensures accurate, current, and safe nursing procedures pertaining to patient care through appropriate supervision of clinical personnel in the clinical area
K. Serves as a resource to SIU staff for SIU Cleft patients

Education 5%
A. Provides patient teaching and participates in staff and community education programs
B. Functions as resource person for general public
C. Assists other members of the SIU Cleft Clinic with preparation of educational presentations

Administrative Duties 5%
A. Assures that tests and surgeries are precertified according to insurance requirements if necessary
B. Participates in development of policies, procedures, and systems to support patient safety and quality care
C. Maintains or delegates inventory of adequate supplies necessary for daily surgery clinic activities and makes recommendations to the Administrator for equipment, commodities, and non-routine supplies
D. Attends and participates in SIU Cleft Clinic staff meetings
E. Close communication and meetings with surgeons as needed

Research Duties 5%
A. Assists with planning and implementation of research projects as related to patient care
B. Follows research protocols when conducting patient research, including obtain informed consent, providing patient care as directed by research methodology, and documentation patient care according to protocol or clinic standards
C. Collects and analyzes data and assists with publishing results as needed
D. Maintain and update SIU Cleft clinic data base

Performs other duties as required or assigned which are reasonably within the scope of the duties described above.


Qualifications

Credentials to be Verified by Placement Officer

  • Eligibility for licensure as a Registered Professional Nurse with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
