Hiring Range:$76,024.00 - $123,468.80 annually *based on experience

About this Position:

This position will float between our 2 Skilled Nursing Units & our Skilled Nursing Facility

Gulf Coast Skilled Nursing Unit:Located at 13960 Plantation Road in Fort Myers, our Gulf Coast SNU is a 75-bed post-acute rehabilitation facility featuring all private rooms and conveniently located adjacent to Gulf Coast Medical Center. The unit provides short-term rehabilitation and skilled nursing care for patients recovering from surgery, stroke, cardiac conditions, orthopedic injuries, and other complex medical needs. An interdisciplinary team delivers individualized care focused on restoring function, promoting independence, and supporting a safe transition home in a patient-centered environment.

Lee Memorial Skilled Nursing Unit:Located within Lee Memorial Hospital (2776 Cleveland Avenue) in Fort Myers, the 18-bed SNU provides specialized rehabilitation and skilled nursing care for patients recovering from illness, injury, or surgery. Physical, occupational, and speech therapists work closely with physicians and nursing staff to develop individualized treatment plans that improve mobility, function, and independence. The unit proudly maintains a CMS Five-Star Quality Rating and is committed to delivering exceptional outcomes through compassionate, patient-centered care.

HealthPark Care & Rehabilitation Center:Located at 16131 Roserush Court in Fort Myers HealthPark Care & Rehabilitation Center is a nationally recognized 112-bed skilled nursing and rehabilitation facility dedicated to helping patients recover from surgeries, strokes, joint replacements, cardiac and pulmonary conditions, and other complex medical events. Through a collaborative interdisciplinary approach, patients receive individualized care and therapy services seven days a week in well-equipped rehabilitation gyms. With a strong focus on recovery, independence, and successful transitions home, HealthPark provides an award-winning environment that supports exceptional patient outcomes.

As a Physical Therapist, you will be responsible for examining, evaluating and testing individuals with mechanical, physiological and developmental impairments, functional limitations, and disabilities or other health and movement-related conditions in order to determine a diagnosis, prognosis and plan of treatment intervention, and assess the ongoing effects of intervention. The PT is also responsible for alleviating impairments, functional limitations and disabilities by designing, implementing and modifying treatment interventions. Additional responsibilities include reducing the risk of injury, impairment, functional limitation and disability, as well as, the promotion and maintenance of fitness, health and wellness in populations of all ages. The PT is engaged in administration, consultation, education and research. The PT is expected to conduct themselves in an effective professional behavior manner. The attributes, characteristics or behaviors required for success in this job include: critical thinking, communication, problem solving, interpersonal skills, responsibility, professionalism, use of constructive feedback, effective use of time and resources, stress management, and commitment to learning.
