SUMMARY:

Provides telephonic triage and document review services that require clinical judgement and interpretation and acts as a liaison between the patient, care team(s), and their primary care provider.

RESPONSIBILITIES:

Reviews and triages telephone encounters, calls, and messages from patients and de-escalates or escalates to the appropriate care team or clinician.

Prioritizes urgent medical triage to patients.

Providestimely assessment for smart scheduling into clinician schedule.

Reviews lab results, imaging results, consultation notes, and clinical summaries and identifies those that need to be directed to the respective clinicians for review.

Effectively communicates with appropriate disciplines involved in the care of patients and provides information and recommendations to providers.

Develops professional working relationships with health care providers and facilities, generating an open flow of information and support of home care goals.

Promotes effective working relationships with nursing personnel, the management team and health care providers.

Determines individual patient needs for services, referrals and training and provides patient education as necessary on plan of care.

Attends required meetings and huddles.

Seeks guidance and resources, when necessary, in care and treatment of patients.

Provides guidance and direction to colleagues and acts as a resource/mentor for nonclinical staff.

Coordinates team based care in the absence of the Program Manager.

Complies with federal and local confidentiality laws, including HIPAA, ensuring patient privacy.

Adheres to Brown Health Medical Group Primary Care guidelines and policies for protecting patients' demographic, clinical and financial information.

Performs other job-related duties as assigned

MINIMUM QUALIFICATIONS:

Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.

Education and Experience

Graduate of an accredited school of nursing

Current Rhode Island RN license in good standing

3-5 years of appropriate nursing experience

Knowledge, Skills, and Ability

Capacity to collaborate with colleagues and teammates as necessary to ensure quality work.

Ability to work both on a team and independently, with the capability to self-direct.

Ability to use technology with proficiency in electronic medical record (EMR) platforms and Microsoft Office applications such as: Outlook, Excel, and Word.

Ability to function well in a high-paced and at times stressful environment.

Capacity to multi-task, think critically and problem solve.

Clear assessment, creativity, judgment, and decision-making skills.

Ability to take initiative, organize, prioritize, and follow through with work assignments assuring fulfillment of plans and goals.

Excellent communication skills - both verbal and written.

Ability to maintain quality, safety, and/or infection control standards.

Ability to perform standard clinical procedures according to established protocols.

Ability to maintain professionalism and provide superior customer service to all.

Physical Requirements

This role works primarily in a remote capacity and has daily communications through virtual meetings, some in the office may be required for meetings, trainings, etc.

Prolonged periods of sitting at a desk or standing while using a computer.

Work and move among all departments of Brown Health Medical Group Primary Care group.

Excellent hand/eye coordination.

Lift up to 15 pounds.

Manual dexterity to operate a personal computer or laptop as well as standard office machines and equipment as it pertains to one's position.

State-wide travel and operation of motor vehicle may be required for offsite meetings and trainings.

Must have a valid driver's license and clear driving record.

Use of personal vehicle and proof of auto insurance is necessary.

Visual Acuity, Hearing, and Speaking.

Capability to transfer information from original source to paper, computer, telephone and in person

Pay Range:

$74,256.00-$148,512.00
