We are seeking a Licensed Vocational Nurse / Licensed Practical Nurse to join our team in our Escondido, CA location.


2nd Shift Position - Sunday-Thursday - 3pm-11pm


Earn $38/Hour -$40/Hour - Depending on experience.


The LVN provides direct nursing care under the supervision of a Registered Nurse and ensures implementation of physician orders, coordination of medical appointments, and continuity of care for individuals with complex medical needs. This is a structured, team-based environment where nurses are supported and empowered to deliver high-quality care.


Key Responsibilities



  • Implement program policy and procedure according to program regulations and guidelines.
  • Provides overall medical support to individuals who receive services to include scheduling medical appointments and follow-up as needed.
  • Works closely with Program Director and Administrator to ensure health and safety of individuals receiving services.
  • Provides on-site medical support to individuals.
  • Represent Merakey at all external meetings, conferences, etc. as assigned.
  • Be knowledgeable and adhere to program regulations, assisting in preparation for annual licensure as applicable.
  • Maintain frequent contact with family members, as deemed appropriate.
  • Maintain frequent contact with external stakeholders as needed.
  • May perform other related duties when assigned or required.



Qualifications



  • LVN/LPN Active CA license in good standing
  • Valid driver's license.
  • 1 year experience in long-term care, developmental services, or skilled nursing preferred
  • Strong organizational and documentation skills
  • Ability to work collaboratively in a team environment



The position is responsible for overall medical needs of assigned individuals in our Intellectual Developmental Disability residential programs.
