We are currently seeking qualified candidates for the following position:

Position: Case Manager Supervisor



The Supervisor of Case Managers ensures that Case Managers and the SSVF Housing Specialist deliver services to clients in a manner that is consistent with professional case management standards, the contractual requirements of the funding sources, and the policies, guidelines, and shared values of VOA.



Supportive Services for Veteran Families (SSVF) is a community-based outreach program, we identify low-income veteran households and connect them with a range of supportive services to promote housing stability and prevent homelessness. Location: Bronx, NY 10451



Minimum Qualifications:



  • Bachelor's Degree in Human Services with three years of experience with a similar population of clients and one year of supervisory experience, or 60 colleges credits with satisfactory years of supervisory experience.
  • Demonstrated ability in verbal and written communication, including basic computer skills, and the ability to work with multiple agencies.



Case Manager Supervisor Principal Responsibilities:



  1. Oversee the performance and development of Case Managers and the Housing Specialist, ensuring that they deliver high-quality services in line with program guidelines, contractual requirements, and organizational values. Provide ongoing training for case managers on new policies, tools, and methodologies to enhance service delivery.
  2. Conduct regular one-on-one meetings and team discussions to ensure that case managers and housing specialists effectively manage caseloads, meet performance standards, and adhere to funder and organization policies.
  3. Provide coaching, mentoring, and professional development opportunities to enhance the skills and knowledge of case managers and housing specialists.
  4. Develop positive linkages, internal and external agency services, generic community resources, and state/city liaisons.
  5. Monitor and ensure the timely and accurate completion of case documentation, including intake forms, assessments, Individualized Housing plans, a Temporary Financial Assistance package, and progress notes.
  6. Conduct periodic audits of case files to assess compliance with regulations, policies, and best practices.
  7. provide direct case management services for clients, including assessments, IHP, service coordination, TFA, and advocacy services.
  8. Ensured the quality of client services by modeling best practices for case management and direct client interactions. Support the housing specialist in providing client-centered housing assistance, ensuring that clients' housing needs are met with a focus on long-term stability.
  9. Evaluate case managers' and housing specialists' performance through regular assessments, providing constructive feedback and addressing performance challenges when necessary.
  10. Document case management activities in a confidential, timely, and concise manner.
  11. Support outreach with screening and assessment efforts to identify veterans who may be eligible for SSVF services. Conduct outreach to veterans enrolled in the program, including home visits and shelter settings. Additionally, perform community outreach to identify veterans who may benefit from the services offered through SSVF.
