Position Title: Senior LTSS Service Care Manager (RN)

Work Location: Remote (must live in Lubbock TX and surrounding area)

Assignment Duration: 6 Months with possibility of conversion or extension

Work Schedule: Monday - Friday 8 am-5 pm CST.

Training Schedule: Monday - Friday 8am-5pm CST - 6 week course classroom setting - CAMERA ON - NO TIME OFF DURING TRAINING. OT unlikely but must be pre-approved. Off during organization holidays.


Work Arrangement: Member-facing - Must live in Lubbock Tx and surrounding area

Position Summary:

The purpose of this team is to provide comprehensive clinical care management and service coordination for members enrolled in STAR Kids and related LTSS programs. The team ensures members receive appropriate, timely, and medically necessary services while maintaining compliance with Texas Medicaid requirements, contract deliverables, and audit standards.

Background & Context:

This need is driven by coverage continuity and operational workload demands for a backfill of a vacant FTE position. The surrounding team is a collaborative, clinically focused group consisting of RNs, Service Coordinators, team leads, and leadership support.

Key Responsibilities:



  • Managing a complex member caseload
  • Conducting clinical and service coordination reviews
  • Completing required assessments
  • Participating in interdisciplinary collaboration
  • Ensuring documentation meets regulatory standards
  • Supporting issue resolution, provider coordination, member and caregiver education
  • Proactive identification of care gaps
  • In-home visits up to 3x a week (must have reliable transportation)


Qualification & Experience:







Candidate Requirements
Education/Certification Required: Associates

Requires Graduate from an Accredited School of Nursing or a Bachelor's degree and 4-6 years of related experience

Bachelor's degree in Nursing preferred
Preferred: Bachelors
Licensure Required: MUST have RN (TX) Preferred:
Years of experience required: 2+ years pediatrics experience, case management, RN required, looking for someone that wants to grow their career at the company.
