The purpose of the Palliative Care RNCM is to support the best quality of life for patients and their families who suffer chronic or acute life limiting illness by reporting a patients physical symptoms, social, emotional., spiritual, or financial condition to the interdisciplinary team. The RNCM provides supportive care for the chronically ill patient or patient who is suffering a life limiting illness and thereby reducing hospitalization when appropriate. The Palliative Care RNCM works under the direction of the Director of Clinical Services

Position Highlights & Primary Responsibilities:

  • Conduct physical assessment to evaluate the clinical and functional status of the patient specific to the diagnosis causing the patient to be referred to palliative care
  • Observe and interpret patient's symptoms and changes in healthcare preference or goals of care and provide update to Primary care provider and specialist via Tiger text, Epic, email or telephone call
  • Inform and engage Palliative care providers who can address gaps in understanding as well as management of symptoms stemming from patients chronic/serious illness
  • Collaborate with physicians, nurse practitioners, social worker, and dietician to devise individualized care plans for patients
  • Provide nursing care/treatment according to physician's orders
  • Assess the patient and families current needs and challenges and communicate them to the team
  • Assess patient and family's current understanding and level of knowledge about patient disease processes and trajectory of illness
  • Work directly with patient, patient surrogate and family to articulate their particular desires for care or treatment in the form of a Serious Illness Discussion developed by Optum
  • Foster a supportive and compassionate environment to care for patients and their families
  • Attend and be prepared for weekly Interdisciplinary Group discussions
  • Expand knowledge and capabilities by attending educational workshops, conferences etc.
  • Meet productivity requirements
  • Document assessment data and care plan within 24hours of visit
  • Participate in on-call rotation weekends/nights/holidays

Required Qualifications:

  • Registered nurse with active unrestricted license in the State of Nevada
  • Current CPR certification
  • 2+ years of varied clinical and community service venues
  • Proven skills in nursing practice, ability to cope with family emotional stress and be tolerant of individual lifestyles
  • Proven good computer skills
  • Proven understanding of alternate delivery systems
  • Proven knowledge of home hospice regulatory and admission requirements
  • Demonstrated initiative toward problem solving
  • Ability to assess patient's physical and emotional status and safety environment and accurately recognize and report identified changes
  • Valid Nevada Driver's license and personal auto insurance coverage
  • Possess a motor vehicle in good working condition that is available during working hours for job related tasks

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.94 to $51.63 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
