We are seeking a dedicated Licensed Practical Nurse (LPN) for a full-time day/evening rotation (5x8s) position. The candidate must hold a valid NY LPN Licensure and BLS certification through AHA or RQI. A minimum of 1 year of experience as an LPN is required, preferably in a long-term care (LTC) facility.


Responsibilities

  • Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit.
  • Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
  • Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
  • Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
  • Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care.
  • Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals.
  • Maintain current knowledge and competency in Nursing Practice through continuing education and in-service programs.
  • Participate in the mentoring/orientation of staff and provide leadership in the professional practice setting under the direction of the RN.


Requirements

  • NY LPN Licensure and BLS certification (AHA or RQI).
  • 1 year of experience serving in an LPN capacity at an LTC facility.
  • Willingness to work every other weekend (EOW) and holidays.


Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
