This role focuses on improving health outcomes by providing support, education, and linkages to resources. The CHW-Care Coordinator will address social determinants by connecting patients to needed services, navigating the healthcare system, accessing and providing adequate support resources. Duties & Responsibilities: Point of contact for patients and community, assisting with care coordination and navigation of health and social service systems. Outreach to establish strong partnerships with families/patients, community organizations, healthcare providers, and local agencies. Partner with Project Manager in recruitment, conducting, and supporting culturally appropriate health education classes and promote healthy eating, chronic disease self-management and/or behavioral health. Conduct needs assessment of the community and clients to connect them with resources in partnership and collaboration with Social Workers ensure continuity of care. Collect data and document ( track and report) all engagement and health status improvements, referrals, and progress in accordance with the program. Attend meetings and ongoing training and professional development sessions to strengthen skills and stay current with best practices. Contributor to Project newsletter Complete other duties as assigned Minimum Qualifications Required: 1. Any one or combination totaling three (3) years (36 months) from the categories below: A. Coursework in social work or behavioral sciences, or closely related field as measured by the following conversion table or its proportional equivalent: 30 semester hours equals one (1) year (12 months) Associate's Degree (60 semester hours) equals eighteen months (18 months) 90 semester hours equals two (2) years (24 months) Bachelor's Degree (120 semester hours) equals three (3) years (36 months) B. Progressively more responsible work experience in social service/ counseling at a paraprofessional level under qualified supervision.
