This role focuses on improving health outcomes by providing support, education, and linkages to resources. The CHW-Care Coordinator will address social determinants by connecting patients to needed services, navigating the healthcare system, accessing and providing adequate support resources.

Duties & Responsibilities:
  • Point of contact for patients and community, assisting with care coordination and navigation of health and social service systems.
  • Outreach to establish strong partnerships with families/patients, community organizations, healthcare providers, and local agencies.
  • Partner with Project Manager in recruitment, conducting, and supporting culturally appropriate health education classes and promote healthy eating, chronic disease self-management and/or behavioral health.
  • Conduct needs assessment of the community and clients to connect them with resources in partnership and collaboration with Social Workers ensure continuity of care.
  • Collect data and document ( track and report) all engagement and health status improvements, referrals, and progress in accordance with the program.
  • Attend meetings and ongoing training and professional development sessions to strengthen skills and stay current with best practices.
  • Contributor to Project newsletter
  • Complete other duties as assigned


Minimum Qualifications Required:

1. Any one or combination totaling three (3) years (36 months) from the categories below:

A. Coursework in social work or behavioral sciences, or closely related field as measured by the following conversion table or its proportional equivalent:
  • 30 semester hours equals one (1) year (12 months)
  • Associate's Degree (60 semester hours) equals eighteen months (18 months)
  • 90 semester hours equals two (2) years (24 months)
  • Bachelor's Degree (120 semester hours) equals three (3) years (36 months)

B. Progressively more responsible work experience in social service/ counseling at a paraprofessional level under qualified supervision.
