PRIMARY PURPOSE

Provides clerical and non-clinical administrative support to the department and Care Management staff. Coordinates discharge referrals to facilitate timely discharges under the direction of the Social Worker and Care Manager.

MINIMUM SPECIFICATIONS

Education

  • High school diploma or equivalent.

Experience

  • Prior experience in a healthcare setting preferred.
  • Must have knowledge of chart scanning system.

Equivalent Education and/or Experience

  • May have an equivalent combination of education and/or experience in lieu of specific education and/or experience as stated above.

Certification/Registration/Licensure

  • None

Skills or Special Abilities

  • Must be able to type 20 words-per-minute with a maximum of three errors.
  • Must be able to successfully pass a Customer Service test as established by Employment Services, with a minimum score of 80%.
  • Must be able to communicate effectively with all hospital personnel.
  • Must be able to compose correspondence independently.
  • Must be able to demonstrate good spelling, grammar and vocabulary skills.
  • Must be familiar with the operations of standard office equipment and healthcare terminology.
  • Must be able to write effectively.
  • Must be able to demonstrate good phone techniques and manners.
  • Must be able to use sound judgment in making intermediate to difficult level decisions.
  • Must have working knowledge of Microsoft Word and Excel.

Responsibilities

  • Communicates daily with internal and external customers regarding status of discharge planning activities assigned to personnel.
  • Documents screening, intervention and follow up to ensure effective communication per established documentation procedures.
  • Manage referrals and communication from a variety of sources such as fax referrals, calls, physicians, care management staff, patients, families, etc. to ensure optimal customer service.
  • Organizes and maintains office files and records in accordance with internal procedures to provide timely and accurate information as needed.
  • Interprets and maintains policies and procedures of assigned unit and PHHS to contribute to consistency of operations.
  • Organizes and expedites the flow of work, initiating follow-up actions as required, to reduce unnecessary interaction with supervisor in routine matters.

Job Accountabilities

  • Identifies ways to improve work processes and improve customer satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals of the department and Parkland.
  • Stays abreast of the latest developments, advancements, and trends in the field by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.
  • Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
