Department:PTSL Ambulatory Pharmacy (Central)Department's Website: Summary of Job Duties:Under direct supervision, the Pharmacy Technician IV for Ambulatory Medication Reconciliation (TOSH Preoperative Clinic Support) will have responsibility for performing and coordinating high-quality medication reconciliations to support perioperative care efforts at TOSH. This position must demonstrate special skills, knowledge, and task completion, including expertise in medication history collection, EHR navigation, medication discrepancy identification, and preoperative medication safety workflows.Medication reconciliations will be completed in advance of scheduled preoperative clinic appointments, with the goal of ensuring an accurate medication list is available for clinical decision-making prior to surgery. This role supports perioperative medication safety by identifying discrepancies, clarifying medication regimens, and escalating clinically significant concerns to appropriate providers and/or pharmacists according to established workflows. The Medication Reconciliation Technician IV will work directly with clinic leadership, perioperative teams, and pharmacy leadership to maintain and improve medication reconciliation workflows, documentation standards, performance metrics, and compliance.
