Salary/Hourly Rate:29.00The Youth Transition Specialist plays a critical role in a multi-year grant funded program designed to support disconnected youth with disabilities as they navigate the critical transition from school to adulthood. These services aim to equip youth with the skills, knowledge, and resources they need to achieve their training, education, employment, and independent living goals. Specialists are responsible for the implementation of instruction in job exploration counseling, workplace readiness training, self-advocacy, post-secondary opportunities, and work-based learning experiences for students and/or youth with disabilities ages 14-24. Develops individualized transition plans, coordinates with schools, families, case managers, and community resources for smooth transitions to postsecondary education or competitive integrated employment. This grant position is expected to end in early 2029. Primary Duties Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. These duties and responsibilities will be rated on the Annual Performance Review.

The primary work environment is an office setting with moderate noise and regular interruptions. The environment may also include a variety of specialized learning settings. The employee will be visiting a wide variety of community settings including but not limited to restaurants, stores, recreation facilities, volunteer sites, schools and employer worksites which can vary with regards to noise level (up to loud) and weather/temperature conditions.
