The successful candidate:

***Shift- M-F 8:00AM-4:30PM. May occasionally include evenings, nights, or weekends when coverage needed. Experience in internal medicine, critical care, hematology/oncology highly preferred.***

In this role, the pharmacist works as part of the multidisciplinary health care team to directly care for patients. They will also provide central staffing coverage, as required.

  • Coordinates all clinical pharmacy functions for the acute care areas as assigned to help achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams.

  • Completes patient profile reviews and participates in daily clinical care rounds, huddles, and meetings as an integrated member of a multidisciplinary team

  • Reviews medication regimens for appropriateness, contraindications, and potential interactions.

  • Helps monitor therapeutic plans and medication treatment.

  • Orders, reviews, and acts upon appropriate laboratory tests and other diagnostic studies necessary to support a patient's medication therapy regimen.

  • Performs pharmacokinetic monitoring, renal dose adjustments, anticoagulant management, glycemic management, pain management, prophylaxis review, IV to PO conversions, and antimicrobialstewardship as necessary.

JOB QUALIFICATIONS:

Education: Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending

Licensure: License in good standing with Arizona State Board of Pharmacy

****PREFERRED JOB QUALIFICATIONS****:

  • Experience: internal medicine, critical care (2+ years inpatient hospital experience or completion of PGY1)

  • Desired experience: hematology/oncology
