0.90 FTE / 36 hours per week.



The Care Team Specialist (CTS) - Clinic Coordinator assists in providing high quality patient care including assisting patients in maintaining their physical and mental well-being. As a Clinic Coordinator, the incumbent is responsible for the performance of CTS duties as listed in the CTS Job Description, assisting with daily scheduling issues, systems planning, projects, clinic operations, participating in the orientation and evaluation of the performance of Care Team Specialist and Clinic Assistant team members, direct supervision of Clinic Assistant staff, and acting as a positive role model for staff. The incumbent works in a collaborative way with the Clinic Manager, Site Chief and Reception Team Supervisor for the assigned site. The CTS - Clinic Coordinator exercises independent judgment and works under general supervision of the Clinic Manager.


Job Specific Minimum Qualifications




  1. Must meet the requirements for a Medical Assistant (MA), a Licensed Practical Nurse (LPN), or an EMT as described below:

    1. Medical Assistants must have successfully completed an MA training program from an accredited school or program and must be eligible for and receive national certification within six (6) months of employment or must have three years of MA experience and have current national certification. Medical Assistants must obtain national certification from one of the following certifying organizations and must maintain certification by meeting CME requirements:

      1. National Healthcareer Association (CCMA); or
      2. American Association of Medical Assistants (AAMA); or
      3. American Registry of Medical Assistants (ARMA); or
      4. National Center for Competency Testing (NCCT); or
      5. By an approved and accredited certifying organization with CME requirements.


    2. Licensed Practical Nurses must have successfully completed an LPN training program from an accredited school or program. In addition, LPNs must have current licensure as a practical nurse in the State of Wisconsin or must obtain licensure as a practical nurse in the State of Wisconsin within six (6) months of employment.
    3. Emergency Medical Technicians (EMTs) must have successfully completed an approved training program and have a valid State of Wisconsin license of one of the following:

      1. Emergency Medical Technician (EMT); or
      2. Advanced Emergency Medical Technician (AEMT); or
      3. Paramedic (EMT-P).




  2. Possession of or eligibility for a valid Healthcare Provider CPR/AED certificate is required. Employee must obtain certification within four (4) months of employment.
  3. Minimum of three (3) years' experience in the CTS role at GHC-SCW.
  4. Ability to travel to various work sites is required. Employee is expected to provide personal transportation. Possession of a valid, Wisconsin driver license if the employee is operating a motor vehicle.
  5. Knowledge of and ability to perform or learn standard clinical procedures according to established protocols and policies are required. Ability to perform or learn routine invasive procedures such as phlebotomy and catheterization in accordance with established clinical protocols is required. Employee will be expected to perform routine invasive procedures with additional training if needed, and within a reasonable period of time.
  6. Knowledge of and/or ability to learn how to properly use and maintain equipment, materials and supplies used during the course of treatment is required.
  7. Knowledge of and/or ability to learn and use computer- based applications such as MS Office, electronic medical record systems and scheduling applications is required. Experience with EPIC is desirable. Must type a minimum of 30 WPM with minimal errors.
  8. Knowledge of or ability to learn outpatient clinical administrative procedures are required.
  9. Knowledge of medical terminology and basic pharmacology is required.
  10. Ability to maintain quality, safety and /or infection control standards is required.
  11. Ability to recognize, identify and report problems to appropriate personnel required is required.
  12. Basic understanding or ability to learn about reimbursement/insurance practices for medical care is required.
