Your Impact

As a Service Support Specialist, you will play a vital role in our service delivery by:



  • Coordinating Care: Managing new referrals and assisting with intake assessments to ensure clients receive high-quality care.
  • Building Relationships: Fostering strong connections with Managed Care Plans, Case Managers, and community partners.
  • Supporting Our Workforce: Assisting with the onboarding and coordination of Direct Care Workers (DCW) from hire to assignment.
  • Ensuring Excellence: Conducting home visits and reviewing service plans to maintain our high standards of care and compliance.
  • Problem Solving: Managing scheduling changes and researching incidents or grievances to ensure continuous service improvement.

Requirements
Qualifications

  • Education: High school diploma or equivalent.
  • Experience: One year of experience in service coordination or delivery is preferred.
  • Leadership: Two years of supervisory experience (or equivalent college studies) preferred.
  • Certifications: CPR/First Aid, Article 9, and Principles of Caregiver certifications.
  • Skills: Proficiency in business software, excellent professional communication, and the ability to manage complex documents and deadlines.
