Responsibilities:

At Plains Regional Medical Center in Clovis, New Mexico, you'll play a key role in supporting patients in our ambulatory care setting. You'll work closely with Care Managers, Medical Providers, and a dedicated care team in an outpatient clinic environment, helping patients with chronic conditions navigate their care. This role also connects patients to community programs, resources, and healthpromotion services. Candidates with clinical experience or a passion for health promotion will thrive here.

• Participate in the planning and implementing, the plan of care as developed by Care Manager.
• Report changes in patient condition as reported by patient to Care Manager and/or Primary Care Provider as appropriate. Support patient and family teaching.
• Maintain a safe environment for patients, families and visitors.
• Keep current with community health care worker standards, PHS philosophy, and PHS programs in order to maintain quality.
• Maintains education development through participation in education programs current literature, in-service meetings and workshops.
• Performs other functions as required.
• Interacts effectively with customers, providing instructions and information, both written and verbal as appropriate.
• Participates in local, regional or state community health care worker events/meetings.

Qualifications:

• High School Diploma/GED required.
• High school plus specialized
• Short-term training in clinical, medical skills .
• Must be able to speak language of the population served.
