Summary
Manages the comprehensive medical nutritional care of patients which includes providing nutrition assessment, development of nutrition care plans, diet modification, nutrition education, and all activities involved in the nutritional needs of the patient population.
Essential Functions
Based on multiple criterion including assessment of nutritional needs, physician's orders, clinical data, interdisciplinary health care plans, and standards of quality nutritional care that appropriately meet age, psychosocial and physical needs of patients, the Dietitian develops and implements nutrition plan of care for patients.
Documentation of all nutrition related information including assessment, nutrition therapy recommendations and care plan, follow up data and changes, diet consultation results, discharge planning, daily clinical activities, and other related orders and pertinent information.
Counsel, educate, and develop resources and materials for the education of patients on families on prescribed therapeutic diets.
Partner with food service staff to ensure meals, supplements, and nutrition support is provided; review patients' menu selections to monitor compliance with modified diets.

Senior Clinical Dietitian

Responsible for designing and implementing a plan of nutritional care for specialized populations, based on identification and assessment of a patient's nutritional needs, correlated with other health plans. Requires strong interpersonal and interviewing skills for competence in both clinical and managerial areas. Requires leadership in a variety of settings and formal and informal teaching to medical students, house staff, nurses and allied health personnel. Serves as resource person in area of specialization for the hospital and community.

PRINCIPAL DUTIES AND RESPONSIBILITIES

Clinical Responsibilities

  • Screens patients for nutritional risk, assesses nutritional needs and develops care plans that include collection and interpretation of anthropometric, laboratory, clinical and nutrient intake data to determine nutritional risk.
  • Employs various methods available for nutrition assessment including the classification of malnutrition based on American Society for Parenteral and Enteral Nutrition (ASPEN) and the Academy of Nutrition and Dietetics guidelines and Nutrition Focused Physical Exam (NFPE) for fat and muscle wasting, edema and micronutrient deficiencies.
  • Develops nutrition care plan utilizing clinical, bio-chemical, and socio-economic factors. Documents and communicates nutrition care plans through written and verbal systems (EMR, discussion on work rounds). Documents in EPIC record based on established standards of practice.
  • As assigned, assesses, recommends and documents on parenterally and enterally fed patients.
  • Interprets, evaluates, and utilizes pertinent current research related to nutritional care, and presents such at Nutrition Support Rounds, as appropriate.
  • Enters accurate and safe initial and renewal TPN orders and ensures MD cosigns. With the guidance of the Dietitian Specialist Group Leader, determines substrate and nutrient formulations including electrolyte management, insulin, H2 blockers, and specialized nutrients (e.g., carnitine). Daily communication of parenteral nutrition prescription changes to the BWH infusion pharmacist.
  • Presents cases and issues at weekly Nutrition Support Service Rounds
  • Adjusts nutrition support care plan with the team based on daily review of any changes in medical conditions, medications, labs and fluid status. Closely monitors adequacy of macro and micronutrient delivery through review of nursing flow sheets and documentation on specialized nutrition tracking tool. With the guidance of the Dietitian Specialist Group Leader, prescribes micronutrient repletion for patients with deficiencies and orders appropriate labs to monitor response.
  • Daily communication of all parenteral nutrition prescription changes to the BWH infusion pharmacist.
  • Participates on multidisciplinary patient care rounds including specialized population (i.e. heart, lung or kidney transplant, ICU, dialysis) and/or has CNSC and enters TPN orders in EMR. Assesses, evaluates and recommends appropriate nutrition intervention in highly specialized patients
  • Collaborates with case managers for discharge and continuing care plans.
  • Assists in the development of standards of practice within the Department of Nutrition. Assists in monitoring of selected clinical indicators and departmental audits.
  • Counsels individuals and groups in specialty area with varied nutritional needs taking into consideration diagnosis, socio-economic, and educational backgrounds.
  • Collaborates with food service personnel and participates in problem solving food service issues.
  • Serves as a nutrition consultant, resource and lecturer for health care providers, students and the general public.
  • Orients new staff to the department.
  • On weekends, in the absence of the Dietitian Specialist Group Leader serves as the lead dietitian.

Additional responsibilities include:

  • Approves or disapproves all TPN initiations in the hospital with support from the Nutrition Support Service attendings
  • Reviews TPN prescription and enters the order for RD's without Certified Nutrition Support Clinician (CNSC) credential
  • Assigns patient load to clinical and senior dietitians to ensure appropriate coverage with limited staffing
  • Reviews all clinical nutrition consults to identify and assign all parenteral nutrition and other high-risk patients

Administrative Responsibilities:

1. Maintains current information on new products and nutrition related product changes and provides meaningful interpretation to physicians, nurses and other health care providers. Assesses nutritional composition and clinical efficacy of products.

2. Maintains food service safety and sanitation standards within established guidelines.

3.Helps controls inventory of nutrition supplements and tube feedings on unit within budgetary goals.

4. Participates in quality improvement projects as requested

Educational Responsibilities:

1. Plans, conducts, supervises, and evaluates the educational experiences of Dietetic Interns for area of specialization.

2.Conducts individual and group classes, both formal and informal, for students and professional personnel, both intra and inter-departmentally.

3. Serves as a mentor to less experiences staff and fosters team work to promote cooperation and support for patient care activities.

4. Educates health care professionals as to the goals of and methods for the

provision of specialized nutritional care to high-risk patient populations.

5. Develops and presents group education classes, in specialized areas to hospital staff, professional organizations, and the community at large.

QUALIFICATIONS

Registered, Licensed Dietitian

Two to three years experience

Certified Nutrition Support Clinician (CNSC) preferred on appointment, Must be CNSC within 6 months of employment/promotion

SKILLS/ABILITIES/COMPETENCIES REQUIRED

1. In-depth knowledge of metabolism and nutrition in relation to specialized area of patient care.

2. Advanced knowledge of food chemistry and macro, micro-nutrient composition for successful alteration of dietary program in a highly specialized population.

3.Knowledge of food production and service systems in relation to safe, nutritious food products for patients.

4.Basic management and personnel skills for interaction with a diverse group of people in the work setting (including physicians, nurses, and other allied health professionals, managers, and supportive personnel).

5.High level of analytical abilities to interpret laboratory data, to identify and plan individual nutritional programs and to discriminate new product claims and their physiologic effect.

6.High level of interpersonal skills necessary to provide counseling in specialization to patients and their families on nutrition related problems.

7.Leadership abilities to conduct programs within the hospital and the community

8.Ability to work in a changing environment.

9.Three to six months of orientation to acquire necessary familiarity with Department and Hospital policies and procedures.
