Under the general supervision of the Y Access Manager, the Transport Logistics Facilitator works in the YNHHS Capacity Coordination Center (CCC) to arrange for emergent and non-emergent medical transportation requests to and from YNHHS facilities. Works closely with the Clinical Bed Managers, Care Managers, clinical staff, hospital administrators and transportation vendors. Provides the, coordination and central communication of the systematic outbound medical transportation. Has the responsibility and accountability for improving the timeliness and efficiency of outbound medical transportation and the satisfaction of internal and external customers. Understands and applies knowledge of appropriate mode of medical transportation, patient throughput bottlenecks, medical transportation vendor capabilities, policies and procedures. Appropriately triages and prioritizes transportation queue and dispatching of health system ambulance resources in coordination with appropriate stakeholders. Must remain focused and calm in the face of multiple competing priorities, and maintain professionalism while carrying out all duties, including relationships with transportation vendors. Must be capable, collaborative, can committed to patient centered care. Must be able to communicate effectively and thrive in a fast paced professional environment with high degree of responsibility. *Potential additional duties include charge reconciliation and prior authorization

Responsibilities

  • 1. Patient Throughput - Obtains appropriate patient census and throughput information from YNHHS inpatient and Emergency Departments. Requires collaboration and communication with delivery network capacity leadership, Bed Management Department, inpatient units, and emergency departments. Utilizes information to triage and prioritize outbound medical transportation in coordination with YNHHS ambulance resources and collaborating ambulance vendors. Responds promptly to requests and changes in transportation need and availability.
    • 1.1. Obtains pertinent patient demographics, medical history, and transportation needs.
      1.2. Obtains input from stakeholders to conduct a needs assessment to inform transportation prioritization.
      1.3. Triages YNHHS ambulance resource requests, prioritizes requests as appropriate.
      1.4. Communicates plans and dispatches according to determined workflows in conjunction with needs assessment.
      1.5. Works collaboratively with all YNHHS sites and transportation vendors
      1.6. Familiar with medical transport contract agreements
      1.7. Knows policies and procedures of pediatric and adult transport teams
      1.8. Responds promptly to requests or changes in transportation status
      1.9. Increases bed capacity by identifying barriers in the discharge process and collaborates with the management team in resolution of these
      issues.
      1.10. Identifies areas of opportunity for process improvement and better utilization of medical transport resources.
  • 2. Customer Service - Utilizes strong interpersonal skills to develop effective relationships with Bed Management, Emergency Department, support staff and ambulance vendors. Communicates clearly and concisely, creating an experience for all that exemplifies caring, understanding, efficiency and overall quality of service.
    • 2.1. Develops effective relationship with all clinical and support staff to facilitate communication and enhance patient flow.
      2.2. Communicates information accurately and appropriately to clinical staff, PFAS management staff, ambulance vendors, patients , coworkers and others.
      2.3. Incorporates stakeholder input into decision algorithm to maximize patient throughput and communicates back to stakeholders.
      2.4. Follows departmental guidelines and identifies events where service was not optimal and initiates and carries through service recovery.
      2.5. Identifies and reports opportunities for improved communication and relations with ambulance vendors.
  • 3. Ambulance Dispatch - Properly obtains appropriate patient information and dispatches YNHHS ambulance resources to maximize patient safety and incorporating knowledge of patient throughput needs and system financial priorities.
    • 3.1. Obtains patient demographics, necessary medical history, location and destination for safe and appropriate ambulance dispatch.
      3.2. Communicates effectively with YNHHS ambulance resources to maximize transportation capabilities and positively impact throughput.
      3.3. Identifies areas of opportunity for improvement and growth of system ambulance resource.
      3.4. Documents and maintains information on volume, and other details as needed by departmental leadership.
  • 4. Resource Management - Maintains strong knowledge of Epic and StatCall applications. Maintains strong knowledge of Connecticut geography and hospital unit and general patient population designations. Strong ability to multitask and integrate information in with working knowledge. Identify and communicate professionally with all relevant stakeholders. Supports organizational and departmental goals.
    • 4.1. Maintains strong working knowledge of all applicable computer systems and dispatch equipment that will affect and support medical
      transportation.
      4.2. Demonstrates strong ability to multitask by utilizing multiple computer systems to support efficient decision making and medical transport
      dispatch.
    • 4.3. Must be able to multitask, interpret and integrate dynamic information and problem solve as needed.
      4.4. Responsible for directing YNHHS ambulance resources safely and efficiently using critical decision-making skills integrated with available
      data.
      4.5. Document and maintain concise and accurate records for patient care, process improvement, program development and other
      departmental goals.
  • 5. Supports Organizational and Departmental Goals - Participates in workgroups, supports team building and communicates with departmental partners. Attends staff meetings, in-services and completes necessary training.
    • 5.1. Attends staff meetings, workgroups, conferences, in-services and other organizational meetings
      5.2. Collaborates and participates in departmental growth activities
      5.3. Participates in the onboarding and training of new employees

Qualifications

EDUCATION

High school diploma or GED required.

EXPERIENCE

Must have a minimum of two (2) years as a EMT or paramedic or a minimum of two (2) years of dispatch experience.

LICENSURE

Active EMT/paramedic certification or International Academies of Emergency Medical Dispatch certification. (Must be obtained within one year of employment) State of Connecticut Telecommunications Certification (Must be obtained within one year of employment)

SPECIAL SKILLS

Ability to direct and triage in a highly fluid dynamic operational environment. Demonstrated ability to manage multiple priorities and assess and triage clinical critical patient needs. Excellent prioritization and communication skills. Excellent critical thinking and conflict resolution skills. Ability to implement, support and effectively monitor change. Ability to collaborate with all layers of the management/administrative/clinical teams within YNHHS and vendors.
