Salary Range Starting at: $20.20/ per hour Job Objective: Provides case management and care coordination services focused on enabling residents to live independently and assume full responsibility and control of their lives; provides counseling services to assist residents, including adults and children, with adjustment to residential living; through individual and group sessions, assesses residents' needs (including adults and children) for additional services. Services include street outreach and continuing support services. What You Will Do: Develops a case plan for each resident within 2 business days of admission to Center; case plan to include planning, including but not limited to the following areas:
Assists residents in implementing the case plan by regular (at least 3 to 4 times per week) individual counseling sessions focused on:
Functions as a liaison on behalf of residents with other agencies; serves as an advocate for residents and clients of the Center as needed. Including but not limited to IPATH, OUTREACH, DVN May be asked to lead educational, instructional, or support groups in one or more of the following areas within the Center
Facilitates residents' adjustment to Center living by:
May be asked to conduct educational, instructional, and support groups in one or more of the following areas in the community:
Offers Life Skills training for all residents for an hour each day
Conducts street outreach to engage individuals experiencing homelessness and connect them with shelter, safety planning, and supportive services.
Minimum Qualifications: The requirements listed below are representative of the minimal education, experience, skills, and/or abilities required for this position. Education: Minimum education of an Associate's Degree along with a plan towards a Bachelor's Degree in a social work or other human services field; Bachelor's Degree preferred; experience in a social service setting providing services to persons in crisis, homeless persons, domestic violence, or mental health preferred. Background Checks: Position requires a background check to be completed, and all background check results will be reviewed. Experience: See minimum qualifications required. Certifications: Must have a valid driver's license and maintain The Salvation Army Driver's qualification standard; must complete Safe from Harm training within the first 90 days of employment. (when appropriate, add: "must complete Caseworker Certification Program within 120 days of employment"). Skills/Abilities:
