HUD Service Coordinator - Hartland/Oconomowoc Area
Location: Hartland, WI (with travel to Oconomowoc)
Schedule: Full-time, 40 hours/week
Employment Type: Onsite, Benefit-Eligible

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a compassionate and resourceful HUD Service Coordinator to support residents across three HUD properties in the Hartland/Oconomowoc area. This role is based at Breezewood Commons in Hartland and includes travel to Hickoryview Commons and Parkland Commons in Oconomowoc (mileage reimbursed).


About the Role:

As a HUD Service Coordinator, you'll empower residents to maintain independence and self-sufficiency by connecting them with vital community resources. You'll serve as a key liaison between tenants and service providers, helping to reduce early admissions to assisted living or nursing homes.


Key Responsibilities:

  • Provide informal case management, intake, and referrals
  • Assess residents' health, psychological, and social needs
  • Develop and monitor individualized service plans
  • Build partnerships with local service providers
  • Educate residents on available services and application processes
  • Coordinate wellness workshops and tenant education sessions
  • Maintain accurate documentation and progress reports
  • Travel between assigned properties (Hartland and Oconomowoc)


Site Breakdown:

  • Breezewood Commons (Hartland, WI): 20 hrs/week
  • Hickoryview Commons (Oconomowoc, WI): 10 hrs/week
  • Parkland Commons (Oconomowoc, WI): 10 hrs/week


Qualifications:

  • Bachelor's degree in Social Work, Sociology, Psychology, or related field
  • Minimum 3 years of experience working with older adults or individuals with disabilities
  • Strong knowledge of community resources and supportive services
  • Valid driver's license and reliable transportation
  • Proficient in written and verbal communication
  • Comfortable using computers for documentation and communication
