Overview

As a member of the interdisciplinary team, contributes professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process. To assess, coordinate screening and stabilization, provide crisis intervention and discharge planning for all behavioral health patients who present to the emergency department. Assist patients and families in accessing community resources regarding long term care, home care needs and/or hospice services. Responds to and assists staff with patients who are victims of violence or trauma to ensure appropriate services and referrals are available.
Qualifications

Educational Requirements: college diploma in nursing
Certification & Licensure Requirements
  • Registered Professional Nurse (RN) License Illinois Upon Hire
  • Basic Life Support (BLS) (AHA) within 30 Days
  • Crisis Prevention Institute (CPI Training) within 3 Months
Other Requirements
Writes, reads, comprehends and speaks fluent English. Basic computer knowledge using word processing, spreadsheet, email, and web browser. Multicultural sensitivity Critical thinking skills using independent judgment in making decisions. Advanced reading, writing and oral skills Ability to utilize PC including, basic elements of Windows environment, keyboarding, work with mouse, and perform basic computer tasks with instruction.

Responsibilities

  • Promotes the use of evidence-based practice through the nursing process, protocols and procedures.
  • Assesses patient's status and identifies risk factors through interview, observation and physical assessment of actual problems and outcomes.
  • Includes significant others in gathering assessment information in order to assist the ED physician, on-call or treating psychiatrists to make the best disposition decision. Obtains release of information when including others in treatment of patient.
  • Provides for a safe environment while the patient is in the ED as well as when the patient is being transferred to another level of care. Serves as CPI resource for the ED and security staff.
  • If a patient becomes agitated, aggressive or violent, CPI utilizes CPI methods.
  • Uses appropriate interventions identified in the plan of care to achieve expected patient outcomes
  • Completes required documentation as appropriate, including patient chart and event reports, voluntary and involuntary admission papers, patient rights, releases, computerized assessment form, discharge safety plan.
  • Demonstrates professional accountability for own practice as evidenced through seeking resources and continuing self-development
  • Demonstrates technical knowledge of equipment and troubleshoots when problems arise
  • Acquires and maintains current knowledge and skills in evidence based behavioral health nursing practice.
  • Promotes the professional development of self and team members.
  • Assures that advanced directives are utilized appropriately: both power of attorney for health care and mental health advanced directives.
  • Incorporates National Patient Safety Goals into the treatment of each patient seen in the ED and serves as a resource for other staff on these goals.
  • Completes all pertinent clinical rating scales (ie, suicide rating scale, etc).
  • Assures that all medical screening assessments, including laboratory diagnostics are ordered.
  • Refers patients appropriately to the most clinically appropriate service(s). Whenever possible, these referrals include communication with the receiving agency while the patient is in the ED.
  • Documents all disposition referrals in patient record.
  • Performs patient bedside lab testing and associated quality control in compliance with medical provider's orders and hospital lab procedures/policies.
  • Coordinates the interdisciplinary care of the patient through collaboration with team members and utilizes additional resources when necessary.
  • Analyzes clinical situations in a systematic way and acts to obtain resources or correct problems to meet or exceed expected outcomes or patient needs
  • Recognizes need for assistance and utilizes resources, including physician, ED BH clinician and nurse to nurse consultation in a timely manner
  • Demonstrates ownership and persistence to ensure problem is resolved in best way possible.
  • Able to assume assignment of non-complex and some complex patient management with turnover in assignment
  • Consistently prioritizes work based on changes in patient condition, changes in assignment and unanticipated interruptions
  • Works with the interdisciplinary team in prioritizing and providing for patient care and safety
  • Identifies and utilizes team resources to solve identified problems or needs , including application of , monitoring of and documentation of restraint/seclusions per MMCI policies, including de-briefing.
  • Provides goal-focused shift and transfer of care report.
  • Advocates with physicians on patient's behalf when necessary
  • Assures that protocols for risk factors are initiated and continued by appropriate staff during patient's stay in ED.
  • Assures that patients with risk factors or other medical concerns are transferred in a safe manner whenever transportation to another facility is needed.
  • Assures that all EMTALA documentation and tasks are completed by the ED team whenever a behavioral health patient is transferred to another facility.
  • Contacts treating behavioral health physician (or case manager/counselor) for assessment information (with appropriate ROI from patient)
  • Contacts on-call psychiatrist (if different from above) immediately following assessment for disposition decision.
  • Coordinates contact between on-call psychiatrist and ED physician whenever necessary.
  • Shares all pertinent assessment data verbally with treating ED physician, psychiatrist and assigned ED RN.
  • Assertively works the various components of the system of care (MMCI and community agencies and services) in order to quickly, efficiently, and appropriately move the patient from triage to disposition, assisting in admissions to inpatient services or making appropriate referrals.
  • Assesses and chooses teaching strategies appropriate to the patient's/family's age specific needs.
  • Adapts teaching strategies based on patient's response, readiness to learn and level of comprehension.
  • Ensures patient safety in the ED environment by identifying risk factors and educating patient, significant others and all team members.
  • Consults appropriate resources within the hospital to provide specialized education
  • Utilizes SVI patient education system as a resource for patients and family members
  • Demonstrates a working knowledge of agencies in city and surrounding counties that serve behaviorally ill, homeless, or other persons who have needs which often present in the ED and educates patients/families/staff.
  • Demonstrates expert knowledge of MMCI behavioral health providers and services and makes referrals when appropriate.
  • Educates behavioral health patients and significant others as appropriate regarding treatment in inpatient units, partial hospitalization programs, or other outpatient services to which they are being referred.
  • Educates ED staff on appropriate interventions in order to promote patient and staff safety when working with behavioral health patients.
  • Actively participates in data collection, analysis, problem solving and gives input on process improvements and patient satisfaction.

Compensation and Benefits

The compensation range for this position is $33.32per hour - $57.31per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
