Minimum to Midpoint Pay Rate:$30.27 - $40.86 / hour

The Cardiac Thoracic Care Coordinator is responsible for managing and coordinating comprehensive care for patients undergoing cardiac and thoracic procedures. This role ensures continuity of care from pre-admission through post-discharge by assessing, planning, implementing, and evaluating individualized care plans. The coordinator serves as a key liaison among patients, families, physicians, and the multidisciplinary healthcare team to optimize outcomes, enhance patient satisfaction, and promote cost-effective, high-quality care.

Education:

• Graduate of an accredited school of nursing (Associates Degree required; Bachelors Degree in Nursing preferred).

Experience:

• Minimum of 3 years of clinical nursing experience required.

• Prior experience in cardiac, thoracic, or critical care strongly preferred.

Licensure:

• Current and active Registered Nurse (RN) license in the State of Florida required.

Certifications:

• Basic Life Support (BLS) American Heart Association required.

• Advanced Cardiac Life Support (ACLS) American Heart Association required.

Skills and Abilities:

• Strong clinical assessment, critical thinking, and organizational skills.

• Excellent communication and interpersonal abilities to collaborate effectively with patients and multidisciplinary teams.

• Knowledge of cardiac and thoracic disease processes, surgical protocols, and patient education techniques.

• Ability to coordinate care efficiently across inpatient and outpatient settings to ensure smooth transitions and positive patient outcomes.
