36/hrs per week. Will work a self-scheduling model with team 3 (12-hour) shifts Sunday-Friday a week.

Job Description Summary:

Coordinates and manages patient care and disposition planning for the Inpatient Psychiatric Unit.

Job Description:

Essential Functions:

  • Coordinates inpatient and post-discharge care, scheduling, and reporting.
  • Provides disposition planning and maintains disposition data.
  • Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
  • Coordinates available NCH and community resources to enhance client care.
  • Provides education and support to patients and their families regarding treatment options and resources.
  • Completes documentation of discharge and care coordination activities in Epic and other related administrative paperwork in accordance with policy standards.

Education Requirement:

Bachelor's Degree in social work, counseling, psychology, or a related field, required.

Licensure Requirement:

LSW, preferred.

Skills:

  • Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients, families, peers and community representatives.
  • Ability to work with the hospital's electronic medical records system.
  • Ability to work and problem-solve with minimal supervision.
  • Ability to adapt to change positively in workflow and process.
  • Spreadsheet, data entry and tracking, and computerized scheduling skills.

Experience:

Experience providing and managing behavioral healthcare to children, adolescent and families, required.

Additional Physical Requirements performed but not listed above:
Demonstrate ability to pass CCM crisis management trainings once trained
