Location:Gulf Coast Medical Center -13681 Doctor's WayFort Myers FL 33912 Department: Pharmacy Work Type: Full Time Shift: Shift 1/9:00:00 AM to 7:30:00 PM Minimum to Midpoint Pay Rate:$51.54 - $67.01 / hour **Eligible candidates can receive up to $7,500 in relocation!** The Transitions of Care Pharmacist plays a vital role in supporting safe and effective medication management as patients transition between hospital, home, and outpatient settings. Key Responsibilities:
Education: PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP). Experience: Five years of professionally responsible pharmacist experience required for Bachelors degree holders. One year of recent inpatient hospital experience as staff pharmacist or pharmacy intern in a general medical/surgical hospital preferred for PharmD candidates. Certification:BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire). ACLS (AHA) and/or PALS (AHA), depending on practice site. License: State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.
