Nurse Practitioner or Physician Assistant Palliative Care

Description

We are seeking a compassionate and experienced Palliative Medicine Nurse Practitioner (NP) to join our interdisciplinary care team. The NP will provide expert medical care to patients with serious, chronic, or life-limiting illnesses, focusing on symptom management, emotional support, and improving quality of life. This role involves collaboration with physicians, social workers, chaplains, and other healthcare professionals to deliver holistic, patient-centered care.

Key Responsibilities:



  • Conduct comprehensive assessments of patients' physical, emotional, social, and spiritual needs.
  • Develop and implement individualized care plans in collaboration with the care team.
  • Manage medications and treatments to alleviate pain and other distressing symptoms.
  • Facilitate discussions around goals of care, advanced care planning, and end-of-life decisions.
  • Provide emotional and psychological support to patients and families.
  • Educate patients and families about disease progression, treatment options, and prognosis.
  • Participate in interdisciplinary team meetings and contribute to care planning.
  • Advocate for patients' preferences and needs within the healthcare system.
  • Maintain accurate and timely documentation in electronic health records.
  • Stay current with best practices in palliative care through continuing education.

Qualifications:



  • Master's Degree in Nursing (MSN) with NP certification.
  • Active NP license in the state of practice.
  • Certification in Hospice and Palliative Nursing (ACHPN) preferred.
  • Minimum of 2 years of clinical experience, preferably in palliative, hospice, or critical care.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal abilities.
  • Experience working in a multidisciplinary healthcare team.
  • Proficiency in patient education and counseling.
  • BLS and ACLS certification.
