Job Summary

The eligible register developed from this posting may be used to fill future FT and PT vacancies within this classification.

This full-time position will support adults with serious and persistent mental illness primarily Monday-Friday during business hours (8:00-4:30) through Targeted Mental Health Case Management. This position may work with the Community Support Program (CSP) and will require some late afternoon/evening hours. The focus of these services is to help individuals thrive in their communities; maintaining stability and maximizing self-sufficiency. This includes gaining access to needed medical, social, educational, vocational, and other necessary services. The Case Manager completes a functional assessment and helps individuals develop a person-centered community support plan, ensures coordination of supports, and monitors the delivery of services. This position is community-based with a Shakopee-based office.


Essential Duties

Screen application for commitment of persons with Mental Illness and/or Substance Use Disorder and provide documentation to the court as required.

Represent Scott Count Pre-petition Screening in court.

Monitors court commitments and compliance of mental health and/or chemical health services and treatments ordered by the Court. Writes reports to the court as mandated

Provides Mental Health Targeted Case Management and direct services for clients in a trusting and therapeutic manner, performing functions required by Federal and State laws and rules.

Works in partnership with individuals to develop individualized case plans; coordinates and directs service to assure implementation of plans.

Evaluates, plans, and implements services for clients.

Performs ongoing case management to include client visits, review of standards, facilitating meetings, identifying progress made toward goals, documenting files and notes, ensuring quality of care, etc.

Assesses needs for clients experiencing homelessness had and help them find housing options

Coordinates with Scott County housing stakeholders.

Supports housing stability by connecting individuals with community resources, monitoring success, supporting good communication with landlords/building managers, and mitigating risks associated with gaining and maintaining housing.

Ensures the safety of clients through the development of support plans, safety plans, and back-up plans; assists in times of crisis to influence positive outcomes.

Assesses, plans, implements, evaluates and collaborates with other professionals and organizations.

Regularly drives a county vehicle to transport self or clients to activities or appointments as the needs of service dictate. Helps arrange/provide transportation for clients.

Provides education, skill development, problem-solving and support to clients and their families to empower and enable them to live independently; advocates for clients to maintain quality services and programs.

Performs professional level case management establishing trusting, helpful, and empowering relationships with clients to assist individuals in success w/decision-making and self-determination.

Prepares and distributes detailed written reports outlining person-centered plans.

Maintains working knowledge of Federal and State laws and regulations and performs work accordingly.

Facilitates Community Support Program activities.

Provides transportation to and from the Anchor Center (CSP).

Assists in developing social, recreational, and educational activities.

Facilitates small groups with various topic areas.

Teaches and role models social skills. Helps clients connect with others and find socialization opportunities in the community.

Works the days and hours necessary to perform all assigned responsibilities and tasks. Primarily M-F 8:00-4:30 for case management. Anchor Center hours are after 4:30 or early evenings. Flexibility in scheduling may be required to meet the needs of the clients.

Must be available (especially during regular business hours or shifts) to communicate with other employees, supervisors, customers, vendors, and any other person or organization with whom interaction is required to accomplish work and employer goals.

Performs other related duties as required or assigned.

Minimum Qualifications

Requires equivalency of a bachelor's degree in a human service-related field and two years of professional social work experience working directly with clients. Master's degree and bilingual abilities are desirable. Must possess a valid driver's license and provide a reliable means of transportation for the performance of work responsibilities.


Selection Process

Selection for this position will be based on a minimum qualifications screening and rating of training and experience. Top candidates will be forwarded to the hiring manager or supervisor for consideration for interview and/or additional assessments. Final selection will include a background check and approval by the County Board.
