Hourly Pay Range:

$40.45 - $62.70 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

  • Position: RN Clinical Coordinator - Cardiac Telemetry 2&8 - Step Down Unit

  • Location: Edward Hospital - Naperville, IL

  • Full Time Position - 36 hour work week

  • Hours: Rotating

What you will need:

Required Education, Skills and/or Experience:

  • Bachelor's Degree in Nursing

  • Five years of clinical direct patient care experience

  • Proficient in Microsoft Office & Epic

Preferred Education, Skills and/or Experience:

  • Master's Degree in healthcare-related field, or currently pursuing

  • Clinical direct patient care of patients within the specialty

  • Continuous improvement experience

Required License and/or Certification:

  • Current State of Illinois Registered Nurse licensure

  • Current CPR Certification issued by American Heart Association or American Red Cross

What you will do:

Coordination of applicable program(s) and other initiatives. Has responsibility and accountability for applicable program(s) and provides clinical and administrative leadership in the care of patients through the care continuum. Coordinates and evaluates operations and clinical program processes to ensure the delivery of quality patient care for patients. Assists leadership with establishing the goals for the program to provide patient-focused quality care, patient satisfaction and cost-efficient care. Direct oversight and responsibility for applicable program regulatory accreditation

  • Responsible for continually reviewing the care of patients in the acute-care setting.

  • Responds to the concerns of patients and their care partners through providing counseling and education pertaining to program(s).

  • Oversees quality improvement processes in collaboration with other program coordinators and leaders. Uses trends in outcomes data, best practices, and patient/provider feedback to guide improvements.

  • Assists in coordinating care of the program patients through collaboration with physicians, coordinators, nursing, case management, social workers, therapists, radiology, lab, dieticians, pharmacy, and all other members of the healthcare team through team meetings and/or one-on-one meetings to ensure a quality care-delivery process for patients across the continuum. Works with this team to identify and track key performance measures including but not limited to patient satisfaction, discharge disposition, and specific outcomes, as necessary.

  • Assists in the development and revision of standards of care, policies, clinical pathways, and order-sets that pertain to the care of patients.

  • Responsible for collecting and recording data pertaining to certification standards. Routinely presents this data to a target audience and engages this audience in achieving benchmark targets.

  • Monitors ongoing documentation of care by healthcare providers for patient populations.

  • Ensures compliance with regulatory and quality standards such as those established by IDPH, the Joint Commission, and other regulatory agencies.

  • Utilizes various methods to increase program-specific awareness to staff. Coordinates with educators in the development of education programs for staff orientation, annual education, and competencies.

  • Works with nursing, rehabilitation, and other services to ensure comprehensive patient education and preparation is achieved and maintained, on an ongoing basis.

  • Works with interdisciplinary leadership to ensure patient education materials are standardized and consistent with order- sets and clinical pathways. Works with appropriate staff to review and approve materials, as necessary.

  • Participates in staff education on an individual and/or group basis. Provides support to the team for continual learning.

  • Works with quality and performance improvement departments to distribute reports to key stakeholders.
