Part Time, Day Shift - flexible hours/days

Summary:

This position implements social service programs according to current federal, state, and local regulations to ensure that the residents' medically related, emotional, and social needs are met and maintained continuously. It also plans and utilizes discharges and helps patients and families adjust to their surroundings.

Detailed responsibilities:
* Social Services Designee

  • Utilize consultation data to assist in planning and coordinating resident care and rehabilitation, following through to ensure service efficacy, and modifying plans to comply with changes in condition.
  • Ensures that a social service assessment is completed with accurate documentation for all new residents within Skilled Nursing and California Department of Public Health Title 22.
  • Performs discharge planning coordination for residents and follows up with discharge planners for residents who have been admitted to the hospital.
  • Monitor, evaluate, and record progress according to measurable goals described in the treatment and plan of care.
  • Identify and assist residents experiencing difficulty adjusting to placements or with behavior problems; refer residents to appropriate resources as needed.
  • Establish involvement with community service organizations for outreach.
  • Assist in developing and implementing policies and procedures for identifying the residents' medically related social and emotional needs.
  • Participate in community planning related to the facility's interests and the residents' and their families' services and needs.
  • Provide families with information on Medicare/Medical and other financial assistance and refer them to appropriate social service agencies.
  • Integrates social services with the other elements of the resident's care plan.
  • Perform all other related duties as assigned.

Education with Equivalent Qualifications:
Essential:
* Bachelor's Degree, with Social Work specialization

Experience with Equivalent Qualifications:

Essential:

* 6 Months Social Services in an assisted living environment

Non-Essential:

* 1 Year Leadership / Supervisory
