Position: Physical Therapist - Start of Care (PT SOC)

Wage range: $90,500-$98,800 annually, depending on experience

Service Area: Twin Cities Metro Area. Plus, mileage reimbursement of .70 cents per mile.

Schedule: Full-Time / Monday - Friday, 8:00am-5:00pm (with 1 holiday rotation and NO ON-CALL & WEEKENDS!)

Primarily responsible to plan, organize, direct homecare services. Experience in physical therapy with emphasis on community health education experience. The professional physical therapist builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. The Physical Therapist is responsible for the implementation of standards of care for physical therapy services.

Position Functions & Responsibilities:



  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness (es).
  • Initiates the plan of care. This may include, but will not be limited to:
  • Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
  • Assesses for muscle strength, mobility, gait, ROM-potential for rehab.
  • Directing physical therapy treatment.
  • Instructing clients and families/caregivers in the use and care of therapeutic appliances.
  • Determining priority needs for physical therapy.
  • Establishes goals based on diagnosis and skilled need
  • Reporting to physician client's reaction to treatment or changes in condition.
  • Includes the client and family in care planning process
  • Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may collaborate with a patient, as well as instructing them as to the goals of the physical therapy program for the client by participating in case conferences.
  • May train client in the use of prosthetic device.
  • Identifies client and family/caregiver needs for other home health services and refers, as necessary.
  • Identifies discharge planning needs as part of the care plan development.
  • Completes necessary admission paperwork accurately and submits them to medical records the day of admission
  • Completes emergency preparedness planning with client/caregiver at admission visit.
  • Communicates with IDT post admission or non-admission. Including client status and summary of the plan of care.
  • Lead by example in establishing rapport with clients.
  • Ensure positive/professional communication on team.
  • Customer service resolution; escalate concerns.
  • Honors client confidentiality, rights, privacy, and reporting maltreatment expectations.
  • Understands and accepts client diagnosis
  • Understands and accepts emotional needs of client.
  • Accepts unique client symptoms and behaviors.
  • Communicates with community health related persons to coordinate plan of care
  • Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition. Obtains/receives physicians orders.
  • Follow all Emergency and Response Procedures
  • Identifies home safety issues and issues cascades info to IDT.
  • Demonstrates proper procedures: Handling, lifting, transfers
  • Notifies IDT of emergency preparedness plan
  • Overviews process, types of reports and forms with client/caregiver
  • Enters complaints, occurrences into EMR as necessary at skilled visits
  • Ensures education on how to use equipment safely is completed and medical supplies are ordered
  • Plots necessary supervisory visits per regulation
  • Plots necessary therapy reassessment visits per regulation
  • Thoroughly reconciles medications at admission
  • Complete all documentation by 10am the day following the admission or skilled visit
  • Complete orientation and all annual training requirements
  • Promote safety in the workplace through real time coaching, participation in safety awareness training
  • Other duties as assigned.



Qualifications:



  • 1-2 years of clinical experience as a physical therapist. 1-2 years of homecare experience required.
  • Graduate of a physical therapy curriculum accredited by the American Physical Therapy Association.
  • Current and active Physical Therapist license in the state of Minnesota.
  • Experience using technology including, Microsoft Office, smart phone/tablet, calendaring, EMR, electronic fax
  • Ability to follow written instructions and type a minimum of 20 words per minute
  • Verification of COVID vaccine status or receiving an approved exemption.
