GENERAL SUMMARY: Supports and promotes the continuum of care for their patient population through telephonic outreach. Provides individualized assistance to patients, families and caregivers to promote optimal outcomes. Works collaboratively with Care Navigation team members, physicians, staff and other providers across the care continuum. Works under the supervision of a registered nurse. PRINCIPAL JOB FUNCTIONS: 1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. 2. *Conducts individual and serial post-hospitalization calls for specific patient populations utilizing established workflows. Consults with RN as needed for patients with complex concerns. 3. *With support and collaboration of the RN Care Navigator, participates in reviewing and modifying the plan of care to support quality outcomes. 4. *Identifies and addresses barriers to timely and appropriate care, connects patients with the appropriate healthcare and community resources. 5. *Routinely updates and provides overview of patient care to the RN Clinic Care Navigators for enrollees in longitudinal care management, transitional care, and other patient panels. 6. *Builds therapeutic and trusting relationships with patients, families and caregivers through effective communication and listening skills. 7. *Provides anticipatory guidance, education and psychosocial support throughout the care continuum. 8. *Maintains patient records accurately, completely, timely and confidentially according to procedures. Documents patient encounters and provided services. 11. *Serves as a resource and provides assistance to the care team 12. * Provides and/or collaborates with staff on patient and family education. 13. *Reports observation of quality issues to the RN and contributes to the performance improvement plan by following recommendation communicated by the RN for program improvement and positive patient outcome measures. 14. *Exhibits behaviors and actions which create a high level of patient/customer satisfaction, which contributes to positive patient/customer relations and which reflect respect for the patient's/customer's rights, needs and confidentiality. 15. *Follows RN guidance to ensure all patient care is provided in accordance with regulatory and safety requirements, which may include Joint Commission, OSHA, state and federal regulations. 16. Assumes responsibility for professional growth and development by staying current in the field by reading related professional literature and undergoing professional educational efforts. 17. *Works within scope of licensure and accesses expertise of RN as needed for care of patients with complex needs. 18. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. 19. Participates in meetings, committees and department projects as assigned. 20. Performs other related projects and duties as assigned. (Essential Job functions are marked with an asterisk "*". Attach Addendum for positions with slightly different roles or work-specific differences as needed. REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: 1. Maintains clinical competency as required for position including but not limited to age-specific competencies relative to patient's growth and development needs and annual skill competency verification. 2. Knowledge of the principles of growth and development over the human life span. 3. Knowledge of computer hardware equipment and software applications relevant to work functions. 4. Ability to establish and maintain effective working relationships with health care team members, physicians and diverse patient populations. 5. Ability to analyze information, understand statistics and provide input into the development of policies and procedures. 6. Ability to communicate effectively both verbally and in writing. 7. Ability to assess data reflective of the patient's status, interpret the needed to identify each patient's requirements relative to their age-specific needs and provide necessary care. 8. Ability to maintain confidentiality of patient and employee information. 9. Ability to prioritize work demands and work with minimal supervision. 10. Ability to provide instruction and direction to others. 11. Ability to maintain regular and punctual attendance. EDUCATION AND EXPERIENCE: Current Licensed Practical Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Minimum of three (3) years recent clinical or patient outreach experience. Experience in self-management support preferred. OTHER CREDENTIALS / CERTIFICATIONS: Basic Life Support (CPR) certification required.
