Support Housing Stability & Recovery at Central City Concern

Are you a passionate, trauma-informed mental health professional ready to make a direct impact on housing stability for Portland's most vulnerable community members?

Join Central City Concern as a Mental Health Clinician and be part of an interdisciplinary team on the front lines of behavioral health, housing support, and crisis intervention. In this unique, field-based role, you'll blend compassionate care, harm reduction, and systems navigation to help clients experiencing serious mental health challenges remain stably housed and engaged in services.

What You'll Do:



  • Deliver rapid outreach and same-day behavioral health interventions.
  • Lead interdisciplinary care planning and crisis support.
  • Help clients navigate complex systems-from housing to detox to inpatient mental health care.
  • Be a connector, educator, advocate, and clinician all in one.


What You Bring:



  • LCSW, LPC, or strong QMHP-level experience.
  • Deep knowledge of harm reduction, trauma-informed practices, and severe mental illness.
  • Ability to work collaboratively across teams and systems.
  • Commitment to equity, social justice, and inclusive recovery pathways.

Salary starting at $65,000 and increases with professional experience and licensure/certifications!

Schedule: Monday-Friday 9:00a-5:30p

MINIMUM QUALIFICATIONS:



  • Must meet qualifications for:


    • QMHP
    • CSWA
    • LPC-a or LPC
    • LMFT-A or LMFT
    • LCSW


  • Must have a Master's Degree.
  • LCSW / LPC HIGHLY Preferred.
  • Experience providing care coordination, case management, or other services to an under-served adult population experiencing mental illness, substance abuse, and/or houselessness preferred.
  • Has the competencies necessary to:



    • Identify precipitating events
    • Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts
    • Assess family, social and work relationships
    • Conduct a mental status assessment
    • Document a DSM diagnosis
    • Write and implement or supervise implementation of a treatment plan
    • Conduct and document a mental health assessment
    • Provide mental health treatment and rehabilitative services within ones scope of practice


  • Current CPR certification required prior to start.
  • Must meet CCC privileging requirements as required by FTCA.
  • Ability to adhere to Central City Concern's drug-free workplace, which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess, or use a controlled substance or alcohol.
  • Must embrace the mission and philosophy of recovery and client-centered treatment.
  • Experience working with dually-diagnosed individuals preferred.
  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
  • Must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum one-hundred thousand/two-hundred thousand dollars in personal auto liability coverage.
  • Must pass a pre-employment drug screen, TB Test, and background check. This includes clearance by the DHS Background Check Unit.
  • Must be able to work variable hours.
  • Must have the physical ability to walk, bend, stoop, twist, kneel, reach, squat, pull and lift up to 25 lbs., climb stairs several times a day and stand for long periods of time. Must adhere to agency's non-discrimination policies.

ESSENTIAL DUTIES AND RESPONSIBILITIES:



  • Provide same-day and next-day outreach, engagement, and brief solution-focused crisis interventions to CCC clients who are identified as needing enhanced behavioral health supports.
  • Provide rapid consultation for supportive housing staff and leadership via dedicated phone tree/line
  • Participate in referral review and prioritization of CCC clients identified as at risk for housing instability or lease enforcement for a reason related to their behavioral health.
  • Support the proactive identification of eligible clients through review of incident reporting data, Collective records, medical records, and other resources.
  • Facilitate participant enrollment into CCC Health Services through completing electronic health record (EHR) registration and program enrollment paperwork.
  • Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent, and for the purposes of performing an initial assessment.
  • Facilitate the development of interdisciplinary, cross-departmental care plans, including client, clinical care team members, and Housing and Supportive Housing care team members in the process.
  • Facilitate interdisciplinary case conferences, including clinical care team members and Housing and Supportive Housing care team members.
  • Leverage systems knowledge and care pathways to support clients in rapidly accessing behavioral health services at the appropriate level of care, including withdrawal management, outpatient therapy, medication-assisted treatment, primary care, and other services as indicated.
  • Coordinate transfer of mental health level of care to meet participant need and provide mental health care as a bridge service while awaiting appropriate level of care availability.
  • Participate in outreach activities necessary to offer services and engage participation in a non-coercive manner. Escort clients to meetings and appointments as necessary.
  • Support clients in accessing needed material resources, including medication, harm reduction supplies, wound care supplies, and other resources.
  • Provide follow-along monitoring of clients following intervention and support re-engagement in behavioral health supports and other services as appropriate.
  • Participate in the development of care pathways, access improvement strategies, and other service design activities.
  • Collaborate with health plans to ensure that clients are able to access available benefits and supports.
  • Collaborate with residential facilities, subacute facilities, and hospitals to support admission planning, care delivery, and discharge planning. Provide services to clients while admitted to inpatient facilities as needed.
  • Create and deliver relevant training, education, and technical assistance to CCC staff, teams, and leaders across our continuum of services, using a variety of instructional techniques such as didactic lecture, role playing, experiential learning, team exercises, group discussions, and multi-media.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

  • Advanced knowledge and skills in counseling and assessment techniques with individuals including ability to perform complex risk assessment and create safety plans for those with complex behavioral health needs.
  • Advanced knowledge in crisis services, mental health levels of care, hospital /ED environments, and complex presentations of severe and persistent mental illness.
  • Knowledge of harm reduction strategies, motivational enhancement therapy, motivational interviewing, and other evidence-based practice interventions.
  • Able to quickly and accurately assess the needs of individual clients, family dynamics and potential barriers to service.
  • Ability to facilitate interdisciplinary care teams, including knowledge of medical systems and terminology and ability to effectively lead case conferences with a broad array of clinical and non-clinical stakeholders.
  • Ability to effectively train and coach others.
  • Advanced communication skills, with the ability to communicate clearly, concisely, and appropriately both orally and in writing
  • Highly adaptable, with the ability to respond flexibly and positively to change and apply past learning to novel situations.
  • Ability to work in an environment subject to ambiguity and change.
  • Ability to learn new skills and have a demonstrated ability to apply principles learned to new situations.
  • Knowledge of de-escalation methods or ability to be trained in de-escalation methods.
  • Ability to display a non-judgmental attitude.
  • Demonstrated knowledge of community and social service agencies.
