Job Description:

Clinical Pharmacy Specialists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication use systems as well as deliver optimal medication therapy to patients within their specialty. All pharmacists are responsible for drug preparation and distribution for medications; evaluating, inputting and verification of medication orders; assisting providers and other health care professionals by serving as a drug information resource; monitoring and managing medication regimens per protocol or per consult; and managing pharmacy inventory.

Job Duties:

  • Provides education to patients, physicians, nurses, and other healthcare team members as requested.
  • MUE, ADR reporting, resolving drug therapy questions, therapeutic drug monitoring, identifying drug interactions and allergies, direct contact with prescribers to clarify major problem orders, and non-formulary drug requests.
  • Responds to medical emergencies in department and throughout hospital as needed including Stroke Alerts and Code Blues.
  • Assist in procedural sedations as well as rapid sequence intubations to ensure safe and accurate delivery of high risk medications.
  • Ensures antibiotic stewardship activities and issues are communicated with the ED practitioners, regarding opportunities to optimize antimicrobial utilization and help reduce inappropriate antibiotic use and antimicrobial resistance.

Requirements:

  • Graduate of an accredited College of Pharmacy
  • Current Kentucky Pharmacist License
  • Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification or within 6 months of start date
  • Completion of PGY-1/PGY-2 ASHP-accredited Pharmacy Residency in area of practice preferred or minimum of 3 years of experience in practice area
  • Current board certification or obtained within first year of hire required
