Responsibilities

Join Our Team as a Continuing Care Planner!

Are you ready to make a meaningful impact in the lives of patients and their families? Under the guidance of our dedicated supervisors, the Continuing Care Planner plays a pivotal role as a liaison for all case management and discharge planning within our facility. Here's a glimpse into the exciting responsibilities that await you:

Essential Duties:

Collaborative Coordination:
Work hand-in-hand with the Clinical Outreach Team to ensure seamless transitions in levels of care and effective release planning.

Family Engagement:
Meet with families throughout the treatment process to demystify continuing care procedures and processes, ensuring they feel supported and informed.

Comprehensive Coordination:
Actively collaborate with the clinical team, utilization review specialists, families, outside consultants, and continuing care facilities to determine discharge dates, level of care recommendations, and transition plans.

Personalized Discharge Planning:
Engage with patients, families, and staff to craft tailored discharge plans and continuing care treatments that prioritize individual needs.

Support Network Development:
Assist patients and families in identifying a robust support network, providing clear instructions for future care, and addressing any follow-up concerns or requests upon discharge.

Documentation Assistance:
Guide patients and families through the completion of all necessary clinical documentation required for a smooth transition to their next programs or social service organizations.

Placement Options Consultation:
Meet with patients and families to explore placement options, organize records for transition, and offer valuable advice on community assistance organizations and transportation options.

Educational Engagement:
Coordinate and actively participate in educational and training activities related to continuing care planning for patients, including exciting travel opportunities to visit programs and facilities both locally and nationally.


Qualifications

  • Bachelor's degree or equivalent experience in behavioral health operations required.
  • Must have experience as a Program Director, Executive Director or Clinical and Operations Manager within an RTC, acute psychiatric hospital, a large psychiatric unit of an acute care hospital or other behavioral health treatment environment.
  • Working knowledge of behavioral health management practices and clinical operations.
  • Working knowledge of state and federal regulatory and various accreditation requirements related to behavioral health management.
  • Working knowledge of operations and financial management and marketing frameworks.
