Under the direction of the Comprehensive Stroke Center Program Manager and in collaboration with the Stroke Program Medical Director, the Stroke Coordinator assists with providing leadership, program management, operational direction, program vision, and strategic initiatives associated with the Stroke Center program. The Stroke Coordinator facilitates the multi-disciplinary care of stroke and TIA patients with the goal to provide positive experience and improved clinical outcomes. Responsibilities include overseeing the regulatory readiness of the advanced Stroke Center by the Joint Commission.

Responsibilities

  • 1. Assists in maintaining responsibility for the clinical and administrative aspects, initiatives and responsibilities associated with the stroke program. 1.1 Assists the stroke program medical director and the stroke program manager with the ongoing management of the stroke program 1.2 Assumes responsibility for the development, ongoing management , quality and outcomes of the stroke program by working closely with the program nurse navigators, and data coordinators 1.3 Serves as a clinical leader and role model for the stroke program 1.4 Maintains an excellent understanding of the medical and nursing knowledge regarding cerebrovascular disease 1.5 Coordinates the clinical care of patients with cerebrovascular disease in concert with the stroke service teams and the neuro-hospitalist teams 1.6 Provides an accurate comprehensive assessment of the relevant medical history, current patient condition and options for coordination of acute and long-term recovery care. 1.7 Assess patient and family understanding of the patient's condition, goal of care and ongoing management 1.8 Provides patients and families with education, regarding their disease process, medications, treatment and plan of care and can serve as a resource for the medical teams and navigators 1.9 Works with Care Management to ensure the efficient transitions of care to rehabilitation facilities and/or community healthcare agencies 1.10 Assists with and in collaboration with the medical director, facilitates and support daily stroke rounds. 1.11Assists with establishing protocols and guidelines for the treatment of acute stroke. Collaborates with the inter-disciplinary team leaders to develop best practices, standards of care, benchmark and treatment guidelines/goals that are evidence-based and cost effective. 1.12 Assists with and facilitates multi-disciplinary administrative and monthly stroke advisory group meetings as required
  • 2. Maintains and prepares for compliance readiness related to ongoing Joint Commission certification standards 2.1 Assists with preparing all necessary certification and re-certification documents and remains current with the stroke program regulatory guidelines, changes and updates as published by the Joint Commission and the American Stroke Association 2.2 Assists with any and all elements of Joint Commission site visit preparation as it relates to both campus reviews and will work collaboratively with the stroke program manager and data coordinators to ensure a seamless site visit 2.3 Participates in the interval intracycle calls and serves as a liaison to the Joint Commission account executive for all communications as warranted 2.4 Serves as a point of contact for YNHH stroke program campuses as needed for local and regional public health initiatives 2.5 Represents - when needed - the stoke program at selected local stroke support groups
  • 3. Assists with the overall stroke program quality improvement process 3.1 Assists with the development of the performance objectives by revising and updating the Annual Performance Improvement Plan 3.2 Assists with educating staff regarding the stroke program's quality improvement process and participates in the review process of data that is submitted to the monthly Brain Matters issues 3.3 Participates in continuous performance improvement activities 3.4 Assists with the coordination of the stroke data analysis and participates in the decisions to improve performance of care and services provided 3.5 Collaborates with the multi-disciplinary team to develop comprehensive plan to systematically document indications, appropriateness, complications and success of treatment 3.6 Maintains a working knowledge of the summary data and uses this information to support the ongoing performance work
  • 4. Collaborates and works with the stroke program faculty, hospitalists, and staff to assess the educational needs of the patients, families and the larger community. 4.1 Assesses needs and collaborates with the physician leadership, nursing educators and members of the inter-disciplinary team to develop and provide ongoing stroke education 4.2 Coordinates and provides education to community providers, emergency responders, and greater community in collaboration with the outreach and integration coordinator and through the Stamp Out Stroke program 4.3 Functions as a resource for providers and community agencies 4.4 Functions as a clinical resource for residents and nursing staff, maximizing opportunities in the clinical setting for informal education of staff, patients and their families.
  • 5. Shares in the accountability for promoting patient and family satisfaction and a positive/centric perception of care 5.1 Supports the program leadership in ensuring that the standards for delivery of optimal care are reflected in the patient and family feedback reports for measuring patient satisfaction 5.2 Provides support to the 7 day follow-up calls and the DPO calls made to patients and families following discharge; tracks the themes that emerge to facilitate improvement initiatives 5.3 Advocates for the needs of bedside nurse so that the delivery of personal and medical care meet the goals of the program's mission statement 5.4 Utilizes the feedback and collection of patient-generated satisfaction data to share information across the health system and throughout the local communities

Qualifications

EDUCATION

Master's Degree required (In Nursing preferred)

EXPERIENCE

A minimum of three years experience in neuroscience nursing, cardiovascular nursing, emergency department nursing, stroke rehabilitation or critical care nursing is required; preferred experience in a leadership position relative to the practice environment.

LICENSURE

CT RN license or CT APRN license

SPECIAL SKILLS

Background in Neurosciences (Neuro-intensive Unit or Neuro-recovery care or Neuro-rehabilitation; must possess a thorough understanding of cerebrovascular physiology, associated pathophysiology and clinical process for care; organizational dynamics, and process improvement. Must be self motivated, goal oriented and able to work independently. Must have the ability to develop relationships and work collaboratively and effectively in partnership with physician, clinical staff, system delivery networks and outside organizations. Must have excellent assessment, communication and organizational skills with the ability to manage and follow-through on multiple priorities. Must have the ability and/or experience collaborating with senior leaders on business strategies and tactics. Excellent critical thinking analysis and assessment skills for successful process improvement planning and monitoring. Position requires recognition as a leader demonstrating excellent interpersonal communication, negotiation, problem solving and customer service skills and ability to lead and motivate work teams.
