The Specialty care coordinator serves as a subject matter expert for resource navigation, and onboarding/training needs. This role reports to the Manager/Supervisor. Essential Duties and Responsibilities Schedules & facilitates patient case reviews; Distributes patient list for review and recommendations to the care team after reviews.
