Report To: Director of Social Ministries Supervises: N/A Pay Grade: 205 Salary Range: $19.97 - $23.96 Qualifications:

Education:
- Bachelor of Science degree in social work, sociology, criminal justice or other related field. In lieu of a degree, candidates with a minimum of five years' experience in social work or case management with a social service organization will be considered.

- Provide case management services to clients to include, but not limited to: determining client eligibility, reviewing intake material for accuracy, assessing client needs, case plan development and compliance, act as liaison to community resources, provide referrals to appropriate agencies, arrange for client transportation and transporting clients as necessary.
- Ensure that all clients have access to a case manager through appointments and walk ins.
- Establish first contact within 5 days
- Meet with clients weekly or as otherwise determined
- Attempt to meet for discharge planning
- Attend community meetings as they are applicable to proper service implementation.
- Complete all Building Blocks Grant assessments (Initial assessment, 2-4 month follow up, and Final assessment) with 3 to 4 clients per month and send documentation to Building Blocks Grant Evaluator.
- Collaborate with local hotel leadership to place clients in appropriate hotels based on client needs.
- Contact Hotels for availability & client placement.
- Track client duration of stay at each hotel.
- Log client hotel stays in agency internal Excel sheets for data tracking and reporting purposes.
- Communicate with local landlords to locate long-term housing for clients seeking permanent housing options.
- Maintain an appropriate caseload as determined by available funding.
- Assess serious personal safety and mental health issues.
- Act as an advocate for clients when needed.
- Maintain an accurate and updated case plan to support stability and self-sufficiency, including short- and long-term goals with related activities and action steps.
- Maintain written/ and/or electronic case files with all relevant client information and advocacy and financial services as necessary. (i.e., court appointments, landlord/ tenant conflicts resolution, etc.). Maintain all client records ethically and confidentially.
- Create and maintain electronic comprehensive demographic data as required on all participants. Must be able to enter all data required in the Salvation Army Information Management System promptly and with a high level of accuracy.
- Maintain a positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices.
- In administering Corps programs, uphold the policies, goals and the mission statement of The Salvation Army.
- Maintain compliance with The Salvation Army's "Safe from Harm" Certification and other mandated programs.
- Other duties as assigned by Corps Officer.
Position Ranking: Regular Full Time, Non-Exempt Physical Demands/ Work Environment: This Position is required to do light to moderate physical work. In order to successfully perform the essential functions of this position, the individual is regularly required to remain stationary for some periods of time, use hands and fingers, communicate, move, occasionally required to position self to stoop, kneel or crouch, frequently required to reach with and use arms and hands as required in using standard office equipment and a PC. Specific vision abilities by this job include close vision, distance vision and ability to focus. The work environment for this position includes an office environment with a low noise level.
