SUMMARY The intern, under the supervision of a senior manager, will play a crucial role in managing operational and administrative processes related to population health while also providing essential resources to patients identified as having social care needs. The intern will be integral in interfacing with team members, other departments at Grady, patients and their caregivers, healthcare providers, and external partners, facilitating connections to resources that address Social Determinants of Health (SDOH). Specifically, they will assist patients who have identified one or two SDOH needs, such as transportation, utilities, food, shelter, or financial assistance. In cases where patients face more complex situations or have multiple needs, referrals will be made to a Community Health Worker or Social Worker for additional support. They will collaborate with the population health team and various Grady departments to develop and implement innovative population health and community benefit strategies, research best practices in community health programs, create content and presentations to facilitate effective program execution, and maintain accurate population health tracking spreadsheets. Their role also involves evaluating population health initiatives through reports, communicating findings, and utilizing those insights to drive quality improvement efforts. Additionally, they will support community outreach initiatives, including building external partnerships, contributing to health promotion events, and participating in community meetings. This position will also aid the hospital-wide community benefit function to meet national standards and public reporting requirements and may encompass diverse projects such as data tracking for SDOH support fund projects, assisting with food as medicine pharmacy prescription programs, coordinating mobile mammogram units, conducting training sessions on SDOH screenings, and contributing to the establishment of a Senior care and resource center.
